About this file
This document outlines Quality Assurance Special Provisions for a federal contract, establishing comprehensive documentation and certification requirements for delivered products and materials.
The Seller must submit Material Mill Certificates from sources such as ASME, ASTM, or SAE, including specific test data on physical and chemical properties with the Seller's signature, subject to Company review. Weld/Braze Inspection Reports must document complete inspection from fit-up through final inspection, accompanied by radiographic films, base metal identification, and filler metal reports. Personnel Certification requires documentation of individuals passing required certification with specified effective dates and recertification procedures; failing personnel must be disqualified and retrained. Process Certification for each shipment must verify compliance with specifications for heat treating, welding, magnetic particle inspection, penetrant inspection, ultrasonic inspection, and surface preparation, listing applicable specifications, performing organization name, date, and authorized signature. Dimensional Inspection Reports prepared by a designated inspection agency must record actual dimensions or "WINTOL" status with unique numerical symbols identifying each dimension. Nondestructive Test Reports must be conducted per specifications with certified personnel and equipment, identifiable to specific items or materials, and containing authorized representative signatures and serial numbers when applicable. Functional Test Reports must demonstrate system operability to design requirements with authorized signatures. Pressure or Leak Test Reports must identify requirements and observed results, identifiable to acceptance criteria. Seller-Supplied Calibrations documentation must list all system instrumentation requiring calibration with records of methods, standards, and personnel involved. The Seller's Certification of Conformance must accompany each shipment, confirming production from Company-furnished material and compliance with all subcontract requirements with authorized signatures. Additionally, the Seller bears responsibility for packing and shipping to ensure undamaged delivery, with two-week advance notice required, and must provide Operating/Maintenance Manuals containing operational procedures, maintenance instructions, spare parts lists, and handling precautions. Nonconforming items must be reported identifying affected items, drawing/specification numbers, and corrective action plans, though Company approval is required for any disposition leaving remaining nonconformity.

Quality Assurance Special Provisions:
- Material Mill Certificate
Material Certification such as those stipulated through ASME, ASTM, SAE, etc., and Material Test Reports shall be submitted identifying the specification to which the material conforms. Specific test data obtained from the original mill source is required and shall include physical and chemical properties and other specified properties and testing.
Certification shall include the signature of the Seller. All reports shall be subject to review by the Company.
- Weld/Braze Inspection Report
A report(s) shall be submitted that indicates the complete inspection of welds or brazes from the initial fit-up stage through the final inspection. Inspection reports shall be accompanied by all radiographic films, base metal identification, filler metal report, etc. All reports are subject to review by the Company.
- Certification of Personnel
The Seller shall document the list of personnel satisfactorily passing the required certification requirements. The effective date for all certifications shall be specified, and each individual shall be recertified at the end of such periods through retesting procedures.
Personnel failing the recertification requirements shall be disqualified from performing that special process or operation and provided with additional training as required for recertification. The Seller's or any lower tier subcontractor's certification programs and procedures shall be subject to approval and audit.
- Process Certification
Each shipment shall be represented by certification for processes covered by specifications such as (but not limited to) heat treating, welding, magnetic particle inspection, penetrant inspection, ultrasonic inspection, surface preparation treatment, etc., stating that processing was performed to specification requirements.
Certification shall list applicable specifications to which the processes conform and the name of the organization that performed them (if other than the Seller), date, and authorized signature of a responsible representative of the Seller. When parts are serialized, serial numbers shall appear on the certification.
- Dimensional Inspection Reports
A document shall be prepared by the designated inspection agency to record (1) actual dimensions or (2) "WINTOL" if dimensions are within tolerance. The methods of inspection shall be identified when actual dimensions are required. Such dimensions shall have a unique numerical symbol adjacent to the dimension to identify the dimension on the dimensional inspection report. All forms and reports are subject to review and acceptance as required by the contract.
- Nondestructive Test Reports
Applicable nondestructive testing shall be conducted in compliance with the provisions of the specifications and subcontract. Personnel and equipment utilized in performance of such tests shall be certified for the type of test performed if required. Seller shall furnish with, or prior to each shipment, reports of such nondestructive examination of material or items furnished. These reports shall be as specified in the subcontract and applicable specification and be identifiable to respective item or material including specific action, joints, or views of the item furnished. These reports shall contain the signature and title of the authorized representative of the organization performing the test. When items are serialized, the serial numbers shall appear on the reports. Reports shall be subject to review by the Company.
- Functional Test Report
This report of subsystems and systems, as installed, indicates the functional operability of such systems to design requirements. Such reports shall contain the signature and title of the authorized representative of the organization performing the tests and shall be sent to the Company for review and acceptance.
- Pressure or Leak Test Reports
Test reports shall be prepared for pressure and leak tests performed. Reports shall list/identify requirements and observed results. Reports shall be identifiable to the acceptance criteria of items submitted and shall meet the requirements of the Agreement and applicable drawings and specifications. Such reports shall contain the signature and title of the organization performing the tests and shall be subject to review by the Company.
- Seller-Supplied Calibrations
All system instrumentation or devices requiring calibration shall be listed. Calibrations to be performed prior to shipment shall be completed and documented. Records of the calibrations including methods and standards utilized and personnel performing the calibrations shall be documented and provided with the shipment.
- Nonconforming Items
The Company expects to receive equipment items, components, materials, and documentation that conform to all codes, standards, specifications, and procedures in the subcontract. The Seller may use its own-nonconformance program to identify, report, and recommend disposition of all non-conformances, but dispositions that would leave any remaining nonconformity must be submitted to the Company for approval.
The request should identify the affected item(s) by name and serial number, citing the drawing and/or specification number and revision number containing the specific requirement that has not been met.
The request should state the number of nonconforming items being reported. The request should include a description of the nonconformity, identifying requirement(s) not met. The supplier may attach a description of the cause, and a corrective action plan and schedule if pertinent.
The issuance and acceptance of such a request in no way limits or affects the warranty provision or other rights or remedies of this agreement. Such a request shall not establish a precedent or obligation to accept existing or future items not conforming to all provisions of the subcontract.
- Seller's Certification of Conformance
A Certificate of Conformance indicating specification, subcontract or applicable drawing and verification numbers and nomenclature, and bearing the authorized signature and title of a responsible representative of the Seller, shall be required to represent each shipment against this contract. The certificate shall read as follows:
A. Items in this contract have been produced from Company-furnished material on ORNL Shipping Order No.
