About this file
This file is a Pricing Schedule template for evaluating quotations on nonpersonal Hospitalist Services at Phoenix Indian Medical Center. Quoters must provide a single fully burdened hourly rate for each Contract Line Item Number (CLIN) covering both day and night hospitalist services across five contract periods (base year plus four option years). The pricing schedule lists ten CLINs with estimated annual hours of 2,000 for day services and 4,000 for night services per period, though these quantities are for evaluation purposes only and do not guarantee minimum staffing levels or full-time equivalents.
All hourly rates must be fully burdened and inclusive of direct and indirect costs, with no separate reimbursement permitted for overtime, holiday pay, shift differentials, travel, lodging, meals, recruiting, credentialing, or administrative expenses. Rates apply to all authorized hours worked including day, night, weekend, and Federal holiday shifts, and shall not include premium or callback rates. Quoters may submit supplemental pricing narratives in accordance with solicitation Section D.6. The Government will use the estimated quantities solely for evaluation and acquisition planning purposes, and reserves the right to verify mathematical accuracy and correct clerical errors during evaluation.

Instructions
The Government will evaluate pricing using the estimated annual quantities identified below. The estimated quantities are provided solely for quotation evaluation, acquisition planning, and contract administration purposes. They do not establish a guaranteed minimum quantity of services.
Quoters shall provide one fully burdened hourly rate for each Contract Line Item Number (CLIN) for each contract period. Hourly rates shall include all direct and indirect costs associated with contract performance as described in Section B of the solicitation.
The fully burdened hourly rates shall constitute full compensation for all authorized hours worked, including services performed during day, night, weekend, and Federal holiday shifts. Separate overtime, holiday, callback, standby, on-call, shift differential, or other premium rates shall not be proposed. The Government will not separately reimburse travel, lodging, meals, overtime, holiday pay, shift differential, administrative costs, recruiting costs, credentialing costs, or any other direct or indirect costs not expressly identified in the awarded hourly rate.
Hourly rates and evaluated prices shall be expressed in U.S. dollars and rounded to two (2) decimal places.
Quoters may submit a supplemental pricing narrative or pricing worksheet in accordance with Section D.6 of the solicitation. In the event of any inconsistency, the Government-issued Pricing Schedule shall govern for purposes of quotation evaluation and award.
The Government may verify mathematical accuracy and correct apparent clerical or arithmetic errors for evaluation purposes.
