About this file

Attachment B is a Nutritional Analysis Form for the Hillsborough County Public Schools (HCPS) National School Lunch Program (NSLP) pizza procurement (RFP 25134-RFP-DST). The form requires proposers to complete detailed nutritional information for 14-inch cheese and pepperoni pizzas, specifying weights of cheese, pepperoni, dough, and cooked slice average, along with comprehensive nutrient analysis per slice. The form mandates reporting of calories, protein, fat (total, saturated, monounsaturated, polyunsaturated), carbohydrates, dietary fiber, sugars, cholesterol, minerals, and vitamins. Proposers must attach a list of ingredients and specification sheets, and provide a signature from the person verifying the nutritional information.

The nutritional analysis form is a critical component of the HCPS pizza contract, which has an estimated annual budget of $2,400,000 and seeks to establish fixed pricing for high-quality, fresh-baked pizzas meeting NSLP nutrition standards. The form supports the procurement's requirements for precise nutritional documentation, ensuring that proposed pizzas comply with federal school lunch program guidelines. Vendors must demonstrate capability through this detailed nutritional breakdown, which will be evaluated as part of the 100-point scoring system that considers nutritional requirements, cost/value, and other performance criteria. The contract, with a potential initial term and up to four additional one-year renewals, emphasizes the importance of accurate nutritional information in meeting the district's food service objectives.
