About this file

The document is a Recorded Program Eligibility Questionnaire issued by the Oregon Department of Early Learning and Care (DELC) for the Early Literacy Success Community Grants (RFA S-58100-00011280). The form is designed to help organizations determine whether their school-age program requires registration with the DELC's Child Care Licensing Division. Organizations must evaluate their program against five specific criteria, including whether they are primarily a group athletic/social activity, focus on a single enrichment activity, are operated by a government entity, have participants aged 14 and older, or provide care while parents are on-site.

If an organization cannot answer "yes" to any of the screening questions, they are required to complete the detailed questionnaire, which requests information such as program name, contact details, county, supported child ages, operation hours, and a brief program description. Upon submission, the Child Care Licensing Division will review the information and either provide guidance on becoming a Recorded Program or send an email confirming the program is not required to register. The form is dated January 2024 and includes provisions for parent cooperative programs, with specific definitions and operational parameters for such organizations.
