About this file
These are FDA Office of Criminal Investigations (OCI) templates for requesting contracted services related to seized property management. The documents establish procedures for two distinct processes: seizure/storage services and manipulation/disposition of seized property.
The Request for Seized Property Services form captures information necessary to initiate contractor involvement in seizing, transporting, and storing property. Required information includes the OCI case number and title, anticipated seizure date, case agent contact information, property location, site agent contact (responsible for contractor coordination), detailed property description, and designations regarding sensitive or hazardous materials. The form requires specification of current container type, approximate quantity, confirmation of whether property can be moved outside the seizing district, and detailed service requirements encompassing seizure execution, transportation specifications (including specialized vehicles if needed), and storage requirements (climate control, containerization, etc.). Unless otherwise specified, contractors will transport all seized property to contracted facilities in Los Angeles, New York, or Miami.
The Request for Manipulation or Disposition of Seized Property form is completed after storage to authorize final handling of seized items. The form specifies the type of action through checkboxes for disposition (remit to owner/violator, authorized agent, or lien holder), sale (export only, quick sale, or interlocutory), transfer (to Treasury Agent, other federal agent, state/local law, or charitable institution), destruction, or manipulation. Required information includes case identification, case agent contact, storage location, property description, sensitive/hazardous material designations, and special instructions. For destruction actions, contractors complete method and location of destruction, with signatures required from the person accomplishing destruction, a witness, and an OCI witness, along with dates and a detailed inventory table listing CIE item numbers, descriptions, and quantities.
