About this file
This document is an Attachment A - Information Requested from vendors for a contract opportunity from the Washington State Department of Corrections (DOC) to provide electrolysis services to incarcerated individuals. DOC is seeking vendors with experience providing electrolysis, particularly pre-operative electrolysis of the genital area for male-to-female patients. Vendors must describe their business profile, background and experience, electrolysis treatment plans, service locations, equipment needs, availability and capacity, and pricing structure. DOC anticipates serving 10-25 individuals per year and requires vendors to be willing to provide services both on-site at DOC facilities and off-site at vendor locations within 50 miles of DOC facilities.
Vendors must provide details on their credentials, such as professional licenses, membership in relevant associations, and Certified Professional Electrologist credentials. DOC will provide necessary supplies and equipment for on-site services. Vendors should indicate their pricing, whether hourly, per treatment, or a hybrid model, as well as any additional charges. DOC is interested in working with small, diverse, or veteran-owned businesses to provide these services.
