About this file

This is a Bidder Response Form for a Fuel System Maintenance and Services and Fuel Tank Cleaning contract opportunity with MassDOT (Massachusetts Department of Transportation). The RFR seeks qualified bidders to provide fuel system maintenance parts and services, as well as fuel tank cleaning across specified MassDOT districts. Bidders must demonstrate at least one year of experience in providing similar materials and services prior to the RFR submission deadline. The contract requires vendors to service multiple geographic areas across MassDOT's district network, with bidders identifying which districts they can cover. Bidders must provide comprehensive information regarding their company experience, customer service approach, delivery policies, and emergency response capabilities.

Pricing submissions are required for multiple service categories, including trip charges calculated by flat rate, mileage, or alternative methods with differentiated rates for standard, on-call, and emergency responses. Tank cleaning costs must be quoted for five tank sizes: 10,000 gallons, 6,000 gallons, 1,000 gallons, 600 gallons, and 300 gallons. Preventative maintenance pricing is required for diesel tank additives in three volume options (1 gallon, 2.5 gallons, and 5.5 gallons) that meet specific performance specifications including contaminant dispersal, water removal, storage stability, corrosion protection, and microbial contamination treatment effectiveness across all diesel fuel types. Bidders must also disclose any bankruptcies, litigations, or contract defaults occurring within the previous five years and detail their return policies and communication protocols for managing service requests and delays.
