Dalecarlia Water Treatment Plant Risk Management Program / 11-1

Section 11 Hot Work Permit Date: June 1999 Washington Aqueduct Division

Risk Management Program

Introduction

The purpose of this section is to describe a consistent control mechanism to prevent fires from welding, cutting, drilling, grinding, soldering, brazing, explosive actuated tool use, hot tar operations, hot plate use, and open burning activities. In order to comply with 40 CFR 68.85, a permit is issued for hot work operations conducted on or near a covered process. This permit documents that the fire prevention and protection requirements in OSHA regulations, 29 CFR 1910.252 (a), have been implemented prior to commencing the hot work operations.

Permit Procedure

A hot work permit request form, Form K-1 included in Appendix K, must be completed by Dalecarlia WTP personnel or outside contractors for any hot work to be performed on or near hazardous material/chemical process and facilities as follows:

• Work on tanks, containers, piping feed systems, or ancillary equipment containing chemicals or fuels.

• Work in chlorine facility rooms or on any part of the nondiluted chlorine system.

• Work within 25 feet of any flammable/combustible material with NFPA fire rating of 2 or greater.

• Wherever confined space entry testing indicates a hazardous atmosphere.

• Wherever a “Hot Work Permit Required” sign is posted.

• Work, where in the Foreman’s judgement that ignition/explosion of chemicals could occur from sparks, hot slag, etc.

The work site Foreman and the WA Safety and Occupational Health Program Manager or WA Project Engineer will review the proposed task and provide authorization that hot work is required. A hot work permit is then issued to the designated maintenance staff or contractor. This permit documents that appropriate fire prevention and protection requirements have been implemented prior to beginning hot work, the object(s) upon which hot work will be performed have been identified, and the dates for this work have been authorized.

The maintenance staff reviews all appropriate safety precautions to be taken prior to performing the hot work with the work site Foreman, including lockout/tagout, respiratory protection, use of personal protective equipment, etc.

When contractors perform work on WA property and lines, the maintenance staff will point out to the contractors where hot work permit procedures are required.

Section 11 Hot Work Permit

Dalecarlia Water Treatment Plant Risk Management Program / 11-2

Normally, hot work permits expire once each individual task is completed and at the end of the work day. A new permit is completed and issued at the beginning of each work day. However, extended contracts may be issued based on work/tasks to be performed.

After the designated maintenance mechanic or contractor completes items 1 through 6 of the Hot Work Permit Request form, the form must be submitted to the work site Foreman for review and signature of approval. The Foreman then passes the form to the WA Safety and Occupational Health Program Manager and/or WA Project Engineer for review and signature of approval. If the work is to be performed in a confined or poorly ventilated space, the Hot Work Permit Request form must be accompanied by a Confined Space Certificate (when applicable) issued and signed by a Confined Space Technician who has performed all tests and procedures required by the Confined Space Entry Program. The approved Hot Work Permit Request form becomes the hot work permit.

One copy of the hot work permit form is assigned to the maintenance staff or contractor who displays it at the work site until all hot work operations are finished. A second copy of the form remains in the hot work permit issuance book maintained by the WA Safety and Occupational Health Program Manager.

Upon completion, the maintenance staff performs any necessary unlocking/untagging, testing, or other activities necessary to return equipment to service and will notify the work site Foreman that operations may return to normal.

1.12 TIME EXTENSIONS FOR UNUSUALLY SEVERE WEATHER

1.12.1 Procedure for Determination

This provision specifies the procedure for determination of time extensions for unusually severe weather in accordance the contract clause entitled "Default: (Fixed Price Construction)". In order for the Contracting Officer to award a time extension under this clause, the following conditions must be satisfied:

a. The weather experienced at the project site during the contract period must be found to be unusually severe, that is, more severe than the adverse weather anticipated for the project location during any given month.

b. The unusually severe weather must actually cause a delay to the completion of the project. The delay must be beyond the control and without the fault or negligence of the Contractor.

1.12.2 Anticipated Adverse Weather Delays

The following schedule of monthly anticipated adverse weather delays is based on National Oceanic and Atmospheric Administration (NOAA) or similar data for the project location and will constitute the base line for monthly weather time evaluations. The Contractor's progress schedule must reflect these anticipated adverse weather delays in all weather-dependent activities.

1.12.3 Impact

Upon acknowledgment of the Notice to Proceed (NTP) and continuing throughout the contract, the contractor will record on the daily CQC report, the occurrence of adverse weather and resultant impact to normally scheduled work. Actual adverse weather delay days must prevent work on critical activities for 50 percent or more of the contractor's scheduled work day. The number of actual adverse weather delay days shall include days impacted by actual adverse weather (even if adverse weather occurred in previous month), be calculated chronologically from the first to the last day of each month and be recorded as full days. If the number of actual adverse weather delay days exceeds the number of days anticipated in paragraph "Anticipated Adverse Weather Delays", above, the Contracting Officer will convert any qualifying delays to calendar days, giving full

SECTION 01 05 00 Page 7 consideration for equivalent fair weather work days, and issue a contract modification.

1.13 WORKING HOURS

Regular working hours shall consist of an 8-1/2-hour period established by the Contractor Officer, between 7 a.m. and 3:30 p.m., Monday through Friday, excluding Government holidays.

1.13.1 Work Outside Regular Hours

Work outside regular working hours requires Contracting Officer approval.

Make application 15 calendar days prior to such work to allow arrangements to be made by the Government for inspecting the work in progress, giving the specific dates, hours, location, type of work to be performed, contract number and project title. Based on the justification provided, the Contracting Officer may approve work outside regular hours. During periods of darkness, the different parts of the work shall be lighted in a manner approved by the Contracting Officer.

1.13.2 Utility Cutovers and Interruptions

a. Make utility cutovers and interruptions during normal working hours.

b. Ensure that new utility lines are complete, except for the connection, before interrupting existing service.

c. Interruption to water, sanitary sewer, storm sewer, telephone service, electric service, air conditioning, heating, fire alarm, compressed air, and water treatment processes shall be considered utility cutovers pursuant to the paragraph entitled "Work Outside Regular Hours." Such interruption shall be further limited to the hours defined for the interruption. This time limit incudes time for deactivation and reactivation.

d. Operation of Water Treatment Plant Utilities: The Contractor shall not operate nor disturb the setting of control devices in the McMillan Water Treatment Plant, including water, sewer, electrical, and steam services. The Government will operate the control devices as required for normal conduct of the work. The Contractor shall notify the Contracting Officer giving reasonable advance notice when such operation is required.

1.14 LIMITS OF WORK AREAS

1.15 CONSTRUCTION SEQUENCING REQUIREMENTS

The following project components and construction activities are expected to impact the operation of the existing McMillan Water Treatment Plant.

This work shall be completed in conformance with the schedule and construction sequencing restrictions defined below.

SECTION 01 05 00 Page 8

1.15.1 Modifications to the Existing Filter Building Spent Washwater Channel

The work included under this project requires two types of modifications to the existing Filter Building spent washwater channel including a new opening in the channel on the south side of the existing filter building and the installation of a new weir gate at the existing point of discharge from the spent washwater channel to the existing mixing chamber.

Additional pipe connection modifications are also required to the existing mixing chamber as described below.

1.15.1.1 Cutting Hole in Spent Washwater Channel on South Side of the Filter Building and Installing Temporary Closure Plate

On the south side of the Filter Building, the Contractor is required to saw cut a new hole in the south wall of the channel (to allow waste backwash water to flow into the new backwash equalization basin) and install a new temporary gasketed steel closure plate. This work shall be completed during a single plant shutdown, of no more than 10 days in duration. This shutdown shall occur during the low demand season between 4 Jan 2021 and 30 April 2021. Work to be completed during the outage includes preparation and cutting the hole, removing associated construction debris and equipment and installing a temporary gasketed steel closure plate over the hole on the south face of the Filter Building. An additional 24 hours will be required for Washington Aqueduct personnel to drain the 12 existing filters to the top of the backwash troughs prior to allowing the Contractor to start this work. Note: It is anticipated that the existing filter valves controlling discharge into the spent washwater channel leak. This issue will be addressed by Washington Aqueduct staff by taking the plant out of service for the duration described above and lowering the water level in the filters as required.

Following construction of the new Backwash Waste Connection Conduit and the Backwash Equalization Basin and Backwash Equalization Pumping Station but prior to start-up of these facilities, the Contractor shall remove the temporary closure plate to place the new backwash equalization basin into service for testing. This schedule for this removal is dependent upon the construction schedule for the facilities listed above but will require a plant outage. This single outage shall be no longer than 1 day in duration and occur during late summer of 2021. Provide minimum 30-days advance notice to Washington Aqueduct to allow the schedule for this plant outage to be confirmed. This shutdown shall be coordinated with other tasks requiring a plant shutdown during this period to ensure that only one plant shutdown is required per year during this period

1.15.1.2 Installation of New 12-foot Wide Weir Gate at the Discharge from the Spent Washwater Channel (i.e., Inlet to the Filter Building Mixing Chamber) and Installation of New 24-inch Backwash Drain Pipe and 30-inch Settled Water Diversion Pipe through the Mixing Chamber Exterior Walls

The installation of the new weir gate at the inlet to the mixing chamber and backwash drain and settled water diversion pipes will also require a plant shut down. As discussed under article 1.15.1.1, it is anticipated that the existing filter gullet discharge valves may leak into the waste backwash channel while the plant is in operation. This issue will be addressed by Washington Aqueduct personnel taking the plant out of service and lowering the water level in the filters as required to prevent discharge to the waste backwash channel. The modifications required to install the new weir gate include the following steps:

SECTION 01 05 00 Page 9

a. Remove the existing east and west mixer drives and below-slab mixer impellor and shaft and saw cut a slot in the top slab of the east mixing chamber to permit the installation of the new weir gate, associated stainless steel anchor channel and concrete curb.

b. Install the new…
