About this file

This is a Past Performance Questionnaire template for NASA Armstrong Flight Research Center's Request for Quote (RFQ) 80AFRC26Q0005. The document is designed to be completed by previous or current customers of an offeror to provide feedback on contractor performance relative to completed or ongoing contracts. Responses must be submitted by email to Sarah Deroche at sarah.a.deroche@nasa.gov on or before June 1, 2026 at 12:00 p.m. EST. The questionnaire explicitly states that completed responses are considered sensitive and cannot be released to the offeror.

The questionnaire requests basic contract information including project title and number, period of performance with option periods, total project value, contractor status (prime or subcontractor), and contract type (firm-fixed price or cost-reimbursement). Respondents must provide their contact information, role on the program, and length of involvement. The performance assessment section contains six evaluative questions addressing the contractor's understanding of customer requirements and staffing consistency; ability to build effective working relationships with associates, subcontractors, and government personnel; problem identification and resolution effectiveness; safety protocol emphasis and mishap investigation responsiveness; management sufficiency for contract performance; and willingness to rehire the contractor. The questionnaire concludes with a five-point overall performance rating scale ranging from Excellent/Exceptional to Poor/Unsatisfactory, with detailed descriptions of performance standards for each rating level.
