About this file

This document is a Contractor Work Surveillance Checklist designed for documenting the placement or removal of empty Rolloff Boxes (ROB) or Vacuum Boxes during federal contract work. The checklist provides a comprehensive inspection framework for government representatives to evaluate contractor performance, focusing on critical aspects such as equipment arrival, container condition, and work execution. Key surveillance points include verifying the contractor's punctuality, equipment completeness (including appropriate container size, plastic liner, and tarp cover), container weight documentation, visual inspection for damage or corrosion, and overall equipment performance and safety.

The checklist is specifically associated with the Puerto Rico Hazardous Waste Disposal Contract (Solicitation Number SP450025R0003), which involves the removal, transportation, and disposal of various waste types including RCRA hazardous wastes, non-RCRA wastes, Universal Wastes, Compressed Gases, PCBs, and PFAS within military installations or within a 50-mile radius of specified pickup locations. The contract is issued by the Defense Logistics Agency and excludes certain waste types like infectious medical waste, ammunition, explosives, and radioactive materials. The surveillance document provides a structured method for government representatives to systematically assess and document contractor compliance with contract requirements during waste container placement and removal operations.
