About this file
This document is a Contractor Work Surveillance Checklist for Industrial Cleaning Services (ICS) related to the Puerto Rico Hazardous Waste Disposal Contract. The checklist is designed for government representatives to systematically evaluate and document contractor performance during waste removal operations. Key surveillance points include verifying the work crew's arrival time, equipment readiness, and operational effectiveness, with specific equipment requirements such as a 3,500-gallon vacuum truck, heated power washing unit, liquid transfer pump, and appropriate personal protective equipment (PPE). The checklist covers critical compliance aspects like truck cleanliness, chemical compatibility, waste removal procedures, and alignment with Resource Conservation and Recovery Act (RCRA) regulations, with spaces for detailed comments and the government representative's signature to formally document the work surveillance.
