About this file
This document is an Acknowledgement, Agreement and Certification of Compliance with Contract Insurance Requirements for federal contract FA6670-26-X-00XX. The form serves as a binding agreement between a contractor and the U.S. government regarding mandatory insurance coverage throughout the contract performance period, including any modifications or time extensions.
The contractor certifies five key requirements: (1) obtaining and maintaining insurance in the amounts and types specified in the contract; (2) ensuring all insurance policies contain endorsements requiring a minimum 30-day written notice to the Contracting Officer before cancellation or material changes that adversely affect government interests; (3) maintaining New York State Workmen's Compensation Insurance or an equivalent reciprocal agreement with another state for the entire performance period; (4) ensuring all subcontractors maintain current insurance certifications meeting contract requirements; and (5) acknowledging that this agreement becomes part of the contract and is legally binding and enforceable. The Contracting Officer accepts and incorporates this agreement into the contract in compliance with FAR 52.228-5 or 52.228-8, as applicable. The document requires signatures from both the contractor (with insurance company information) and the government Contracting Officer (914 MSG/PK).
