About this file

This is a Vendor Performance Disclosure Form required by the State of Colorado's Office of Economic Development and International Trade (OEDIT) for the National Guard-to-Aerospace Talent Transition Initiative contract opportunity. The form is a mandatory submission that vendors and bidders must complete and return to the OEDIT Purchasing and Contracts Office prior to performing work under any Commitment Voucher or related subcontracts. The document implements compliance requirements under Colorado Revised Statute 24-102-206, which mandates that prospective vendors disclose the geographic location where services will be performed and identify whether any work will occur outside the United States or the State of Colorado.

The form requires vendors to disclose whether services under the contract or any subcontracts are anticipated to be performed outside the United States or Colorado, and if so, to identify all countries and states where work will be performed. Additionally, vendors must provide a detailed explanation of why it is necessary or advantageous to perform work outside the United States or Colorado. The form must be signed by an individual authorized to represent the bidder, with electronic signatures accepted, and submission with the bid is mandatory. This disclosure mechanism ensures state oversight of contract performance locations and establishes a record of vendor justifications for any out-of-state or out-of-country work arrangements.
