About this file
This is a Specialized Experience Questionnaire template used to document and evaluate contractor performance on federal contracts.
The questionnaire is divided into two parts. Part A requires the contractor to provide company information (UEI number, firm name, address, point of contact details), specify their role (prime contractor, subcontractor, joint venture, or other), identify the percentage of project work performed, and provide contract details including contract number, title, location, award date, completion date, and contract price. Contractors must also describe how their project is relevant to NWS sterile insect dispersal or air transportation requirements, including details on similar aircraft, crew qualifications, requirements, and conditions. Part B requires the client to complete contractor performance evaluations using an 11-item rating scale that assesses compliance with contractual requirements, quality standards, timeliness, workforce qualifications, equipment resources, on-site management, safety practices, and overall performance. The rating scale includes five performance levels: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Not Applicable (N), with detailed definitions for each category. The client must sign and date the completed questionnaire and may provide additional remarks to support their evaluation ratings.
