This is a bulk sample analysis report for asbestos testing conducted by Mountain Laboratories for the Mann-Grandstaff VA Medical Center's Café & Kitchen Floors project. The analysis was performed for customer Chris Warner (Customer #1123) located at 4815 N. Assembly Building 32, Spokane, WA 99205, with testing completed on February 20, 2026.

Six samples were analyzed using EPA/600/R-93/116 and 40 CFR Part 763, Subpart E, Appendix E test methods. The samples consisted of floor tiles (samples 1A and 1B), floor grout (samples 2A and 2B), and terrazzo flooring (samples 3A and 3B). All six samples tested negative for asbestos presence, with no detectable asbestos types or percentages (N.D.). The non-fibrous material composition varied by sample type: floor tiles contained 100% other materials with black layers containing 60% quartz and 40% other materials; floor grout samples contained 60% quartz and 40% other materials; and terrazzo samples contained 15% quartz and 85% other materials. The laboratory notes that negative PLM (Polarized Light Microscopy) results should be confirmed with SEM or TEM testing and that sample results cannot be used to claim product certification or endorsement by NVLAP, NIST, or any federal government agency.
