About this file
This file is a Subcontractor Consent Letter template for use in proposals submitted to the Department of the Air Force in response to Solicitation FA330026R0022 for eMentor Foreign Language Course Instructors. The document serves as a template that major subcontractors and teaming partners must complete and submit as part of the Past Performance Volume to authorize the Government to disclose their past and present performance information to the prime contractor during the source selection process. The letter format allows subcontractors to grant consent for the Government's Past Performance Team to discuss their performance history with the prime contractor, facilitating the assessment of performance relevancy and confidence in evaluation.
This is a supporting document rather than a substantive requirements document. Subcontractors and teaming partners are required to provide signed consent forms using this template as part of their participation in the proposal response. The consent mechanism is necessary because Federal regulations restrict disclosure of subcontractor past performance information to private parties without explicit authorization. Completed forms should be submitted with the proposal's Past Performance Volume for the Air Education and Training Command's source selection process.

SUBCONTRACTOR CONSENT LETTER
SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF PAST AND PRESENT PERFORMANCE INFORMATION TO THE PRIME CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government's Past Performance Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past Performance Volume (or Proposal). (Note: RFO 52.212-2, Volume 3, Past Performance should specify if past performance is to be submitted as a separate proposal volume).
