About this file

This document is Attachment 6: Required Bidder Certifications related to a Request for Response (RFR) issued by the Executive Office of Health and Human Services (EOHHS) for a preferred supplier of Durable Medical Equipment (DME) incontinence products for the state of Massachusetts. The RFR seeks a single preferred supplier to provide incontinence products that meet EOHHS's minimum quality standards at a discounted rate to participating MassHealth DME Providers, including MassHealth Pharmacy Providers with a DME specialty. The document outlines five required certifications that the bidder must complete, including certifications related to contract readiness, debarment or suspension, key personnel, financial condition, and use of subcontractors.

The pricing for the incontinence products will be provided through a preferred wholesale distributor arrangement, though specific pricing terms are not included in this document. The RFR does not mention any set-asides, incumbents, or funding requirements. The contract term is not explicitly stated, but the document indicates the preferred supplier will be required to provide the incontinence products for the term of the agreement, including any renewals.
