Solicitation FA8003-23-R-0003 Attachment 6: Past Performance Questionnaire The 771 Enterprise Sourcing Squadron of the Air Force Installation Contracting Center (AFICC) is in the process of competitively selecting sources for Geospatial Support & Services (GS&S) 2.0 program. The GS&S 2.0 program is predominantly professional services. The program also includes tasks associated with USAF Geospatial Agency Program Management and/or Geospatial Program Office Operations Support; Requirements Elicitation (Collection, Development, Analysis, and Synthesis); Geospatial Strategic Support; Communications, Education, and Training Support; Geospatial Data; Geospatial Analysis and Data Presentation Support; Geospatial IT System Support and Management; System Assessment and Authorization; IT Portfolio Management Activities; Application Development, Modernization, and Sustainment; Helpdesk Services and Customer Support Services; and Surge/Contingency Support.

One of the considerations in proposal evaluation is the verification of the Offerors’ past performance on contracts or other work efforts which reflect the Offeror’s ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the Offeror’s performance on those contracts or work efforts.

Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 01 May 2023.

The offeror who has nominated you as a Point of Contact for this effort has completed Section 1 below. In order to provide a complete assessment, please complete Section 2 through Section 6 in the attached Past Performance Information Sheet.

To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to frank.batz@us.af.mil.
