About this file

This is a Past Performance Questionnaire (PPQ) template used by the U.S. Senate Sergeant at Arms to collect reference information during source selection for a hoteling software replacement solution (RFQ 2026-R-062). The questionnaire is classified as SOURCE SELECTION INFORMATION and must be protected as procurement sensitive information.

The form requires offerors to identify a past performance reference by providing contract/task order details including the offeror/subcontractor name, contract number, contract type, project name, period of performance, and brief description of work. References must complete Section B by providing their company/agency name, contact information (name, phone, email, signature), role in managing the contract, and time period of involvement. References then rate contractor performance on eight evaluation criteria using a six-point scale (EX—Exceptional, VG—Very Good, S—Satisfactory, MG—Marginal, US—Unsatisfactory, N/A—Not Applicable). The criteria assess: fulfillment of contract requirements and specifications; timely delivery of goods and services; problem identification and corrective actions; expertise and quality of personnel; personnel selection, retention, and replacement; overall program management; cost forecasting and management; and overall performance. The questionnaire also includes yes/no questions regarding willingness to contract with the vendor again and space for additional performance comments. Completed questionnaires must be submitted within five business days to acquisitions@saa.senate.gov, Attn: Mashonda Walston, Senior Procurement and Contracting Specialist.
