About this file

The document is a Contractor Work Surveillance Checklist for Industrial Cleaning Services (ICS) related to CLINs 1002SV-1004SV and 1007SV-1009SV. The checklist is designed to systematically evaluate contractor performance during industrial cleaning work, with specific focus on equipment readiness, compliance, and work quality. Key items assessed include: crew arrival time, equipment verification (vacuum truck, power washing unit, liquid transfer pump), weight ticket documentation, equipment inspection, chemical compatibility, waste residue management, equipment functionality, and overall work satisfaction. The checklist provides a structured method for government representatives to document and assess the contractor's performance, with spaces for detailed comments and an official signature and date.
