Solicitation No: FA4830-20-Q-0010 Requirement: Mess Attendant Attachment 6

CONTRACTOR INFORMATION

Prior to potential contract award we will require the following information:

1. a. Bank Information—provide the name of your bank and its telephone number; your account number;

and a point of contact. If your banking facility requires a written request for information or charges a fee for the information, as some do, please contact them and have them send the information to us. If you have provided this information within the last year for this contract and there have been no changes, including the contact point at your bank, you may state “no changes.”

b. Current Dun & Bradstreet Rating—provide your company’s rating and any information concerning your rating. If your rating has changed within in the past year, please provide an explanation for the change. If you have previously provided this information for this contract and there have been no changes, you may so state.

c. Equipment List—provide a list of equipment to perform job or resources to obtain equipment. If you have provided this information within the past year for this contract and there have been no changes, you may so state.

2. Current Business Commitments: This is to identify your current business commitments and is expressed in general terms as in the example below. Detailed information for all of your current business commitments does not need to be provided, though detailed information for some of your current businesses commitments will need to be provided as requested in item 3 below. Include all current existing projects/contracts and all bids/proposals for which you are being considered. This should be expressed both in numbers, dollar amounts, and stage of completion.

For example: 5 current open-ended contracts. 3 each basic and 2 years, all for not to exceed $1.5 million total years. 2 of these are in the basic year, 1 in the last option year. 2 each basic and 4 years, all for not to exceed $2.5 million total years. 1 is in the 1st option year and the other in the 3rd option year. 15 additional specific project contracts totaling $15 million. 1 specific project contract for $500,000 pending award. 2 solicitations for open-ended contracts, each basic and 4 years, one for not to exceed $1 million which, if awarded, will start early next calendar year, and another not to exceed $2.5 million, which, if awarded, will start later this calendar year.
