About this file

This is a Quality Assurance Surveillance Plan (QASP) Matrix that establishes performance standards and inspection requirements for a naval contract to restore and upgrade the Secondary Propulsion Unit Rotary Deployment Mechanism (RDM) at Portsmouth Naval Shipyard. The QASP defines nine key deliverables with 100% compliance requirements: RDM refurbishment and upgrade per specified procedures with zero deviations unless approved; hardware replacement of 24 studs and 24 nuts with complete objective quality evidence (OQE); in-shop assembly testing with comprehensive test reports; rotary joint refurbishment with detailed documentation of internal conditions and seawater contamination; training motor upgrade and repair; breakaway torque testing with a maximum limit of 30 foot-pounds; material procurement with certification documentation; and timely submission of all required documentation within 30 days of component shipping. All deliverables require 100% inspection verification to ensure completeness, accuracy, and traceability.

The QASP incorporates a five-level performance rating system ranging from Exceptional to Unsatisfactory. Satisfactory performance—the expected baseline—requires meeting all contractual requirements with minor non-conformances and timely corrective action. An Exceptional rating would demand flawless execution, proactive communication with researched solutions, and early delivery. Marginal ratings result from significant non-conformances such as failure to meet critical parameters like breakaway torque or repeated delays in documentation submission. Unsatisfactory ratings apply to multiple significant failures or critical uncorrected deficiencies that jeopardize project completion. This matrix serves as the government's surveillance tool to monitor contractor performance throughout the execution of the RDM restoration contract.

Sheet1 Quality Assurance Surveillance Plan, Matrix

Deliverable or Service RequirementPerformance StandardsAcceptable Quality Level (AQL)Method of Surveillance
A. RDM Refurbishment & UpgradeRefurbish/upgrade the RDM per Reference 2.2 Enclosure 5 and Repair Procedure RP10748.100% compliance. Zero deviations from referenced procedures unless approved by PNSY via DFS/LAR100% Inspection of Final Report and Objective Quality Evidence (OQE).
B. RDM Hardware ReplacementProvide OQE certifying replacement of 24/EA studs and 24/EA nuts per Para 3.1.1.1.1.100% compliance. OQE must be complete, accurate, and traceable.100% Inspection of submitted OQE documentation.
C. RDM Assembly TestingPerform in-shop testing per Reference 2.2, Para 6. Provide complete OQE for each test performed per Para 3.1.1.2.1.100% compliance. Test results must meet or exceed all requirements specified in the reference.100% Inspection of all submitted test reports and data.
D. Rotary Joint RefurbishmentRefurbish the Rotary Joint per Reference 2.3.100% compliance. Zero deviations from the referenced procedure unless approved by PNSY.100% Inspection of Final Report and OQE.
E. Rotary Joint Disassembly DocumentationDocument internal conditions, seawater contamination, and components replaced with pictures as required by Para 3.1.2.1.1.100% compliance. Report must be detailed and clear.100% Inspection of the initial disassembly report.
F. Training Motor Upgrade & RepairUpgrade and repair the Training Motor per Reference 2.2 and Repair Procedure RP10748.100% compliance. Zero deviations from referenced procedures unless approved by PNSY.100% Inspection of Final Report and OQE.
G. Training Motor Breakaway TorqueDocument the final as-released hydraulic motor worm shaft breakaway torque per Para 3.1.3.2.2. The torque must be within the maximum allowed limit.100% compliance. The documented torque must not exceed the maximum allowed (30 FT-LBS).100% Inspection of the final test documentation.
H. Material Procurement & CertificationProcure all identified materials and provide OQE for all new material utilized per Paras 3.1.4.1 and 3.1.4.2.100% compliance. OQE must be provided for all new material.100% Inspection of material certification package.
I. Deliverables & ReportingSubmit all required documentation to PNSY and SUBMEPP within 30 days of component shipping dates via trackable carrier per Section 8.100% compliance. All reports must be submitted on time, be complete, accurate, and legible.100% Inspection of all deliverables for timeliness and completeness.
RatingsDescriptive Rating System
ExceptionalPerformance meets all contractual requirements and exceeds many to the Government’s benefit. Problems are rare, and the contractor takes highly effective corrective action. For this SOW, an "Exceptional" rating would require flawless execution, proactive communication of potential issues with well-researched solutions, and delivery of all components and documentation ahead of schedule.
Very GoodPerformance meets all contractual requirements and exceeds some to the Government’s benefit. Problems are infrequent, and the contractor demonstrates effective corrective action. For example, all technical requirements are met, and the contractor provides exceptionally clear and well-organized documentation that facilitates rapid government review.
SatisfactoryPerformance meets all contractual requirements. This is the expected level of performance. Any non-conformances are minor, and the contractor's proposed corrective actions are timely and effective. The work is technically sound, and all deliverables are submitted on time.
MarginalPerformance does not meet some contractual requirements. A "Marginal" rating may be assigned if a significant non-conformance is identified (e.g., failure to meet a key test parameter like breakaway torque) or if required documentation is repeatedly late or incomplete. This reflects a need for increased government oversight.
UnsatisfactoryPerformance does not meet most contractual requirements, and recovery is not likely in a timely manner. This rating would be assigned for multiple significant failures, a critical uncorrected deficiency, or a failure to provide required OQE, which jeopardizes the entire project.
