About this file
This is a Quality Assurance Surveillance Plan (QASP) Matrix for noxious weed survey and treatment services. The document establishes performance standards and quality metrics for contractor deliverables, specifically focusing on daily treatment records and electronic data collection and submission activities.
The QASP defines three quality rating levels tied to payment outcomes. Rating 1 (Acceptable) requires 90% or better treatment efficacy on targeted plants, 20% or less loss of non-target species, complete and accurate treatment records submitted on time, and electronic data meeting 90% accuracy or greater within specified timeframes—resulting in full payment. Rating 2 (Deficient) encompasses treatment efficacy between 80-90%, requiring rework at contractor expense; if standards are not met after a second inspection, a 20% payment reduction applies. Failure to submit treatment records or electronic data by specified deadlines, inaccurate records, or electronic data below 90% completeness results in a 10% payment reduction. Rating 3 (Unacceptable) applies when services do not comply with contract terms, do not meet Quality Standards for Ratings 1 or 2, or fail to provide any treatment records and electronic data—resulting in no payment if performance falls below 80%. Monitoring methods include weekly review of quality control plan documents by the Contracting Officer's Representative (COR), periodic onsite inspections by designated inspectors and/or the COR, final efficacy inspection at least 14 days after completion using ocular estimate and monitoring plots as needed, and office review of treatment records and electronic data by the COR.
