About this file
This document is a Quality Assurance Surveillance Plan (QASP) for a federal contract opportunity to provide a combined 75-25 didymium oxide powder, also known as neodymium-praseodymium oxide (NdPr Oxide). The contractor must package and deliver up to 172 metric tons of NdPr Oxide to the Defense Logistics Agency Hawthorne Army Depot in Nevada over a one-year period. The QASP outlines the required services, including packaging, labeling, sampling, and testing requirements, as well as the quality assurance methods the government will use to monitor the contractor's performance. It specifies 100% and random inspections, with the contracting officer's representative (COR) completing monthly inspection reports. The contractor must meet the acceptable quality levels (AQLs) defined in the Performance Requirements Summary Table to receive a "pass" rating. Failure to meet the AQLs may result in the contractor having to re-perform the service.
The related federal contract opportunity is a solicitation (SP8000-24-R-0003) issued by the Defense Logistics Agency for the mining, processing, and delivery of up to 172 metric tons of NdPr Oxide. This is an unrestricted, firm-fixed-price, lowest-price technically acceptable acquisition, with all NdPr Oxide required to be sourced and processed exclusively in the United States, its outlying areas, or Canada. The solicitation will have a 12-month period of performance, with an estimated closing response date of June 24, 2024.

Perform the following specific service(s);
1) The Contractor shall perform all service(s) to satisfy the above listed “Required Service” and any additional service(s) required by the SOW.
Primary Methods of Surveillance: 100% and Random inspections.
Frequency of Surveillance: The COR will perform inspections in accordance with paragraph 9 of the SOW. Based on observed defects in contractor performance, subsequent inspections may either be reduced or increased. At a minimum, the COR will complete a “Government Inspection Report” monthly. (See Attachment 4 to this SOW for a copy of the “Government Inspection Report”).
Acceptable Quality Level (AQL): All service(s) shall be performed at or above the Acceptable Quality Levels (AQL’s) listed in the Performance Requirements Summary Table (PRST) of this SOW.
Method of Surveillance: The method of surveillance will be random unless listed differently in the Performance Requirements Summary Table (PRST) of this SOW.
Surveillance Procedures: 100% inspections or Random inspections will be performed on scheduled service(s) unless listed differently in the Performance Requirements Summary Table (PRST) of this SOW. There are no special procedures to be followed to initiate unscheduled inspections.
Evaluation Procedures: The COR will observe the selected service(s) at any time they determine a need to inspect a service(s) or notices a deficiency. Results of each inspection will be documented on a “Government Inspection Report”. (See Attachment 4 to this SOW for a copy of the “Government
Inspection Report). A “pass”, “fail” or “N/A” rating will be assigned to each service inspected based on the Performance Criteria listed below. A brief description of the inspection and observed defects will be recorded. A “fail” rating may require contractor re-performance of service(s). The COR will ascertain whether documented defects were corrected, and processes implemented to ensure re-occurrence of the defect is prevented. An “N/A” rating will be assigned when no work was required to be performed on a specific service during that rating period.
Performance Criteria:
1) Each “Required Service” requirement may contain several services. Each requirement will be evaluated based on an individual service.
2) At a minimum, the COR will inspect all service(s) listed in the Performance Requirements Summary Table (PRST) of this SOW at the frequency listed in the PRST.
3) A “pass” rating will be assigned to each service inspected that met or exceeded the established Acceptable Quality Level (AQL) for each “Required Service” requirement evaluated.
4) A “fail” rating will be assigned to each service inspected that fell below the established Acceptable Quality Level (AQL) for each “Required Service” requirement evaluated. When a “fail” rating is assigned, all observed defects must be documented on a “Government Inspection Report”. (See Attachment 4 to this SOW for a copy of the “Government Inspection Report”).
5) An “N/A” rating will be assigned when no work was required to be performed on a specific service during that rating period.
Analysis of Results:
1) The COR will notify the Contractor within 48 hours each time a “fail” rating is assigned and discuss the basis for the rating.
2) The COR will solicit the Contractor’s proposed corrective action(s) to preclude re-occurrence of the defective service(s) performance.
3) Based upon the Contractor’s performance, the COR may recommend corrective action(s).
Subsequent inspections may be performed more frequently if defects continue.
4) The COR will document the results of each inspection using a “Government Inspection Report”. (See Attachment 4 to this SOW for a copy of the “Government Inspection Report”). The COR will complete a “Government Inspection Report” monthly at a minimum. The COR will upload copies of all completed “Government Inspection Reports” into the SPM Application within the Procurement Integrated Enterprise Environment (PIEE), the web-based system.
