About this file
This is a Past Performance Questionnaire (PPQ) used by the 60th Contracting Squadron at Travis Air Force Base, California to evaluate contractor performance for a 60 OSS Air System Maintenance requirement (Contract Number FA442726Q1106).
The questionnaire is completed by customers of the contracting company being evaluated and requests detailed information across three main sections: contractor identification information, respondent/customer information, and contract performance data. Respondents must rate the contractor's performance on a 1-5 scale across six dimensions: quality of service, on-time delivery of service, management of service, communication, contract terms and condition adherence, and overall performance. Additionally, respondents indicate whether the contractor received any Corrective Actions Reports (CARs) or similar deficiency reports, and whether they would award another contract to the contractor. Narrative explanations are required for any ratings of 1 (Unsatisfactory), 2 (Marginal), or 5 (Exceptional). The completed questionnaire must be submitted as a PDF to TSgt Hunter B. Losen, Contract Specialist, at hunter.losen@us.af.mil by August 26, 2026 at 12 PM PDT, with confirmation of receipt available by calling 707-424-7736.
