About this file

This is a Past Performance List of References template form used by federal contractors to document their previous contract experience and qualifications.

The template requires vendors to provide detailed information on a minimum of three past performance references organized in a tabular format. For each reference, vendors must supply the contract number (including task order numbers if applicable), a description of work performed, the contract period of performance, the contracting agency point of contact with name, phone and email, and total contract values broken down into both total contract amount and annual amount. Additionally, vendors must disclose whether the work was performed as a subcontractor, affiliate, or joint venture partner, and if so, specify the percentage of effort contributed. The form includes spaces for three separate contract references, each with identical data collection requirements. This document serves as a standardized method for federal contracting officers to evaluate vendor experience, capability, and past performance on similar or relevant projects during the procurement evaluation process.
