About this file
This document is a NASA Past and Present Performance Questionnaire for Solicitation 80KSC026R0007, which involves a Launch Complex (LC) 39 Area Duct Bank System Replacement project. The questionnaire is designed to evaluate a Design-Build Company's (DBC) performance across six key areas: overall performance, conformance to contract requirements, timeliness and schedule adherence, business and management practices, quality of work, and an overall rating. Respondents are asked to rate performance levels ranging from Exceptional to Unsatisfactory for each category, with space provided for explanatory comments. The form also includes a final question asking whether the respondent would work with the firm again. The completed survey is to be emailed to Ben Crafton and Jacqueline Brooks at NASA, with an option to reference a Contractor Performance Assessment Reporting System (CPARS) report in lieu of filling out the entire questionnaire.
