About this file
This file is a Lowest Price Technically Acceptable (LPTA) Technical Acceptability and Price Submission Template for a construction project. It serves as the required submission format for quoters responding to RFQ No. 140P5326Q0017 issued by the National Park Service Southeast Region for the cosmetic repair of a pedestrian bridge foundation at Mammoth Cave National Park, Kentucky.
The template requires quoters to demonstrate technical acceptability across five mandatory evaluation criteria to remain eligible for award. Quoters must provide relevant experience through two completed projects within the past five years involving concrete repair and/or masonry/veneer installation on structural concrete elements, including crack and spall repair, epoxy injection, or stone/block veneer installation. The scope of work includes performing concrete crack and spall repair on approximately 663 square feet of bridge foundation surfaces using epoxy injection and polymer-modified repair mortar per ASTM standards; beneath-bridge substructure repairs with epoxy-coated rebar dowels and 5,000 psi low-shrink concrete; installation of sandstone veneer and retaining wall blocks matching existing materials per ASTM C1780 and NPS historical standards; development of a site-specific Accident Prevention Plan complying with OSHA requirements; and coordination of utility locates and temporary protections with completion within 180 calendar days from Notice to Proceed. Additionally, quoters must identify any specialized subcontractors, confirm compliance with all SOW/specifications requirements through a detailed checklist, submit a proposed construction schedule demonstrating completion within 165 calendar days, and certify their agreement to maintain continuous pedestrian access with ABA/ADA compliance and implement fall protection systems to prevent debris displacement onto infrastructure below.
