Attachment 5 – Evaluation Information

The Government intends to evaluate proposals and award a contract without discussions (except clarifications as described in FAR 15.306(a)). Therefore, the offeror’s initial proposal should contain the offeror’s best terms from a price and technical standpoint. This is a competitive Request for Proposal (RFP) that represents the best value to the Government conducted under FAR Part 15, Lowest Price Technically Acceptable (LPTA) source selection process.

Award will be made to the responsive and responsible, lowest priced, technically acceptable Offeror that meets or exceeds the solicitation requirements upon availability of funds.

Evaluation Overview: Proposals will be evaluated and ranked according to price, from lowest to highest.

The Government reserves the right to evaluate only the technical proposal of the lowest priced Offeror.

Other technical proposals may not be evaluated unless the Government determines the lowest price Offeror to be unacceptable or non-responsive. In the event the Government determines the lowest price Offeror to be unacceptable, the technical proposal of the next lowest price Offeror will be evaluated. This process will continue until a technical proposal is determined to be acceptable

Offers will be evaluated utilizing the following evaluation criteria:

Volume I – Factor -1, Technical Capability

The offeror shall demonstrate its ability to meet the requirements of Section 5 of the PWS and its ability to provide adequate staff to complete the requirements.

Volume II: Factor 2 - Past Performance

Offerors may submit up to four (4) past performance references on past or current contracts for efforts similar to the requirements outlined in the solicitation, with Government instrumentalities (Federal, State, or local) or private industry utilizing the Past Performance Questionnaire (Attachment 6 – Past Performance Questionnaire). Offerors shall complete Section 1 of Attachment 6 – Past Performance Questionnaire and send (Attachment 6 – Past Performance Questionnaire), to the reference specified in Section 1; this shall be completed for each past performance reference. Offerors shall request that each reference complete Section 2 of (Attachment 6 – Past Performance Questionnaire) and return it to the Coast Guard in accordance with the directions in Section 2. It is the offeror’s responsibility to ensure the reference contact information in Section 1 is valid. If the reference does not submit a Past Performance Questionnaire, the reference may not be considered in the evaluation of the offeror’s past performance.

The offeror shall complete Section 1 of the past performance questionnaire (Attachment 6 – Past Performance Questionnaire) and return the questionnaire to the Government with Volume II.

The offeror’s similar past performance must have been performed within the last three years or be ongoing. The offeror’s past performance may include information regarding predecessor companies or subcontractors that will perform major or critical aspects of the requirement, if applicable. Offerors may include information on problems encountered on the identified contracts and the offeror’s corrective actions.

Volume III - Factor 3: Price Proposal

Offerors shall submit their price using Attachment 7, Pricing Sheet and enter their price in worksheet “Price.” Offerors shall enter their price for each Contract Line-Item Number (CLIN). Only cells C3 through C8 shall have a price entered. Cell C9 includes a formula that adds cells C3 – C8. No other cells

Attachment 5 – Evaluation Information shall be changed or adjusted. Attachment 7, Pricing Sheet “Estimated Level of Work” provides an estimated level of work for each option year.
