About this file
This is a Past Performance Questionnaire template designed for vendor references to evaluate contractor performance on past projects relevant to the Science and Technical Support Services (STSSC) solicitation (RFP 140G0226R0003) issued by the Department of the Interior US Geological Survey Office of Acquisitions and Grants. The document is structured in two parts: vendors complete Section 1 with project details (title, location, contract number, dates, and amounts) and client/agency information, then submit the entire questionnaire to their past performance references for completion. References must return completed questionnaires to Lisa D. Williams, Contract Specialist, no later than 2:30 p.m. Mountain Daylight Time (MDT) on July 27, 2026.
The questionnaire evaluates contractor performance across nine criteria using a five-point rating scale: Outstanding (O), Above Average (A), Satisfactory (S), Marginal (M), and Unacceptable (U). References assess the contractor's relationship with the client team, corporate management and integrity, quality control, schedule performance, any cure notices or terminations, willingness to award future contracts, customer satisfaction with final products and services, and opportunities provided for the contractor to respond to negative comments. An overall performance rating is required, with space for supporting comments. This template applies to the 100% competitive 8(a) set-aside solicitation (NAICS 541990, size standard $19.5 Million) under FAR Parts 12 and 15.
