About this file

This is a Past Performance Questionnaire template for NOAA solicitation 1305M326Q0374 concerning Woods Hole Facility's Cottage Building Exterior Siding, Trim, and Paint work. The template serves as an assessment instrument to evaluate contractor performance on current or historical contracts relevant to the proposed effort.

The questionnaire is structured for completion in four sections: Sections I and II are completed by the offeror and major subcontractors, then submitted to a government assessor with relevant contract experience; Sections III and IV are completed by the assessor to document their role and provide an overall performance rating. The assessor must verify the contractor and contract information provided in Sections I and II, identify their involvement role (Procuring Contracting Officer, Administrative Contracting Officer, Contract Specialist, Contracting Officer's Representative, Technical Project Lead, Quality Assurance Specialist, or other), and assign an overall performance rating using a five-point scale: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable. The assessment considers contract requirement fulfillment, quality and timeliness of delivery, cost management, communications, proactive management, and customer satisfaction. Completed questionnaires must be submitted to the Contracting Office contact, Jenna Taulman, at jenna.taulman@noaa.gov, with the offeror also submitting copies of Sections I and II with its proposal by the solicitation closing date.
