About this file

This is a Past Performance Data Sheet template used by DLA Disposition Services to evaluate contractor performance on previous contracts relevant to the Large Area Maintenance Shelter (LAMS) Repair/Maintenance solicitation (SP4510-26-Q-1013).

The document is a two-part questionnaire where Part I is completed by the contracting company and forwarded to government or commercial references. This section requires contractors to provide contract numbers, points of contact, detailed work descriptions, contract dollar values, subcontractor information, and documentation of any quality, delivery, or pricing problems and their resolutions. Part II is completed by the reference organization and returned directly to DLA Disposition Services to provide an independent assessment of the contractor's past performance. Respondents evaluate the contractor across five major performance categories: Technical Performance (service compliance, deliverable accuracy, and problem resolution); Management/Key Personnel (adequate staffing and responsiveness); Schedule Compliance/Timeliness (delivery schedules and submission timeliness); Regulatory Compliance (environmental, hazardous waste, bio-based products, and EEO regulations); and an overall performance rating. Evaluators use a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory) with specific definitions for each rating level and must provide supporting narrative for all assessments. The questionnaire concludes with a yes/no question regarding whether the reference would award to the contractor again.

Exceptional Performance meets contractual requirements and exceeds many to the Government's benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

Very Good Performance meets contractual requirements and exceeds some to the Government's benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.

Unsatisfactory Performance does not meet most contractual requirements and recovery in a timely manner is not likely. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
