About this file

This file is an attachment containing guidelines and instructions issued by the South Carolina Department of Juvenile Justice for preparing a Pre-Adjudicatory Waiver Evaluation Summary Report. The document serves as a comprehensive template and checklist for County Case Managers to systematically document family and community information regarding youth involved in Family Court proceedings. The report must be submitted to the Consultation and Evaluation Services Administrator within 10 calendar days of the Family Court order for waiver evaluation. The guidelines require detailed assessment across multiple categories including family composition and history, parental backgrounds, marital status, family dynamics, social and religious activities, behavioral patterns, health and mental health information, economic status, home conditions, neighborhood characteristics, and prior involvement with social agencies.

The second section of the guidelines addresses the youth's history and background, requiring documentation of previous court contact, DJJ services rendered, physical descriptions, developmental history, home adjustment, religious participation, physical and health history, psychological and psychiatric information, school performance and records, recreational interests, and employment or vocational history. The document mandates that case managers obtain supporting documentation including immunization records, health insurance information, school records, standardized test scores, special education plans (AD504), psychological assessments, and discharge summaries from mental health providers. This evaluation summary report functions as a critical information-gathering tool to support judicial decision-making regarding waiver evaluation proceedings in South Carolina's juvenile justice system.
