About this file

This is a tailored evaluation criteria addendum for commercial products and services under FAR Part 12 simplified acquisition procedures. The Government will evaluate quotations using three primary factors: Technical/Quality, Past Performance, and Price, with comparative evaluation methodology to determine best value rather than lowest price. The Technical/Quality evaluation will assess the proposed 1.5 Tesla mobile MRI system, clinical capabilities, equipment compatibility with the Tomah VA site, delivery and installation approach, maintenance and technical support capability, ability to meet emergency response and 98 percent monthly uptime requirements, OEM support, training approach, and IT/cybersecurity compliance. Quotations exceeding minimum requirements may receive favorable consideration if they provide meaningful benefits to the Government.

Past Performance will be evaluated based on recency (within five years of solicitation closing), relevancy to mobile MRI systems or similar medical imaging equipment leasing and support, and quality of performance, with consideration given to CPARS data, customer surveys, and other reasonable sources; quoters without relevant past performance will not be evaluated unfavorably solely on that basis. Price evaluation will include all line-item prices and options, with total evaluated price calculated by adding the base period, all four option periods, and six additional months priced at the Option Year Four monthly rate under FAR 52.217-8, though quoters submit pricing only for base and option periods. The effective date of this evaluation addendum is January 15, 2026 (Revision 03).

ATTACHMENT 3: TAILORED 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES ADDENDUM to 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (DEVIATION AUG 2025)

(a) The following factors shall be used to evaluate quotations:

(1) Technical/Quality

(2) Past Performance

(3) Price

(b) Evaluation Approach. The Government will evaluate quotations using comparative evaluation in accordance with the Simplified Acquisition Procedures of RFO FAR Part 12.

Quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest priced quotation that provides additional benefit(s).

Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each quotation must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations.

The evaluation will consider the following:

(1) Technical/Quality. The Government will evaluate the extent to which the quotation demonstrates that the proposed mobile MRI system, mobile coach, equipment, services, and performance approach meet the requirements of the solicitation and Statement of Work. The evaluation will be based on the information requested in Attachment 2, Instructions to Offerors.

The Government will comparatively evaluate the proposed MRI system and clinical capabilities; required equipment and accessories; compatibility with the Tomah VA site; delivery, installation, commissioning, and readiness approach; maintenance and technical support capability; ability to meet the required emergency response, twelve-hour restoration, forty-eight-hour replacement-unit, and 98 percent monthly system uptime requirements; OEM support, parts, updates and upgrades; training approach; and compliance with applicable IT, PACS, remote diagnostic, and cybersecurity requirements.

Quotations that exceed minimum requirements may receive comparative consideration where the additional capability provides a meaningful benefit to the Government in performing the requirements of the Statement of Work.

(2) Past Performance. The Government will evaluate past performance to assess the relative risk associated with a quoter's likelihood of successful performance of the solicitation requirements as indicated by the quoter's record of past performance.

The Government will consider the recency, relevancy, and quality of performance of available past performance information. For purposes of this solicitation, recent performance is performance occurring within the five (5) years preceding the solicitation closing date. Relevant performance includes the lease and/or support of mobile MRI systems or other medical imaging equipment of similar complexity, scope, and support requirements.

The evaluation may consider information provided by the quoter; the Contracting Officer's knowledge of and previous experience with the services being acquired; Contractor Performance Assessment Reporting System (CPARS) information; customer surveys or questionnaires; and any other reasonable source.

A quoter without a record of relevant past performance will not be evaluated favorably or unfavorably solely on that basis.

(3) Price. The Government will evaluate price by adding the total of all line-item prices, including all options. The Total Evaluated Price will be that sum, as adjusted for evaluation of FAR 52.217-8 in accordance with paragraph (c) below.

(c) Options. The Government will evaluate quotations for award purposes by adding the total price for all options to the total price for the basic requirement. This includes the option under FAR 52.217-8, Option to Extend Services, which applies to this solicitation.

Evaluation of the option under FAR 52.217-8 will be accomplished by adding six (6) months of the quoter's price for the last potential period of performance under this contract to the quoter's total price. The Total Evaluated Price will therefore include the base period, all four option periods, and an additional six months at the monthly price applicable to Option Year Four.

Quoters shall price only the base and option periods specified in the contract line-item numbers (CLINs) and shall not submit a separate price for the potential six-month extension of services under FAR 52.217-8.

The Government reserves the right to exercise the option under FAR 52.217-8 before the end of any of the contractor's performance periods and the option will be exercised at the price and/or rates in effect at the time the clause is exercised. Evaluation of options shall not obligate the Government to exercise the option(s).

(End of Provision)
