SUMMARY OF WORK
PART 1 – GENERAL
1.1 SUMMARY
A. The intent of the contract is to provide for the complete construction of the project as described in the contract. The project requires the contractor to be responsible for supplying all labor, materials, equipment, miscellaneous items, freight, delivery, and all necessary valid licenses and permits required to complete work identified in the construction drawings and project specifications.
B. The US Forest Service (USFS) plans to make improvements at the Rexford
Campground Complex. The original campground complex (campgrounds, day use beach, boat ramp, trails, etc.) was built primarily in 1979 in conjunction with construction of the Libby Dam and creation of Lake Koocanusa. Original work was completed by the US Corps of Engineers. Both water and sewer are connected to the Town of Rexford systems through agreement with the USFS.
C. Some past wastewater system repair/replacement work has been completed. These past projects were a response to problems that began to occur with the aging system. Primary problems are reportedly the galvanized force mains are leaking and the Lift Station No. 1 has had operational and flooding issues. This project aims to address these past problems and others more permanently, with a water and wastewater focus.
D. Water & wastewater improvements at the Rexford Campground Complex will be implemented in one Base Bid and two Additive Alternate Items. Work also includes replacing the single-phase electrical power main with a 3-phase conversion. This will require Contractor coordination of electrical work with Lincoln Electrical COOP.
The main Rexford Bench Campground Complex project components in the Base Bid consist of about: 1687 LF 3-inch HDPE force main; 664 LF 1.5-inch force

main; air relief vault; Boat Ramp Host site electrical, water, and wastewater (278 LF PVC sewer service) utilities; clean existing sewer 3889 LF 4 and 6-inch PVC gravity lines; repair 6-inch sewer (165 LF at 6 locations); two package lift stations; approach manhole; utility building; and flow meter vault. Additive Alternates (Optional Work Items) include 1) reconstruct existing RV Dump Station with all new components, earthwork, concrete formwork and flatwork, asphalt pavement, and electronic fee pay station; and 2) water system improvements with remote read flowmeter, electrical service, and water sample station.
E. Rexford Campground Complex is located about 8 road miles (~12 minutes) west of Eureka Montana. The campground entrance is off Highway 37 and Gateway Street. The boat ramp and day use areas are on the shores of Lake Koocanusa.
1. The campground footprint is about 120 acres, and the lot size is 217 acres.
The legal description is North ½ Section 12, Township 36N, Range 28W.
The acreage is owned by the US Government (USFS). Lake Koocanusa borders to the north, highway 37 is the south border, more USFS property borders to the east, and the west border is the town of Rexford and USFS more property.
2. The property address is 316 Rexford Bench Road, Rexford Montana, 59930.
3. The property Owner address is the USA, 1101 US Highway 2 West, Libby Montana, 59923.
F. The site may have features of historic significance, and possible archaeological and/or cultural significance. As necessary a Forest Service provided archaeologist will be available during excavation phases of the project to observe and record any findings. The Forest Service archaeologist will coordinate with the Contracting Officer (CO) and the Contractor may be required to stop work in the event of a historically and/or archaeologically significant finding.
1.2 MEALS AND LODGING
A. Meals and lodging are commercially available in Eureka, Montana.
1.3 FIELD VERIFICATION

A. Field verify all dimensions affecting the work of this contract before ordering products.
B. Provide site survey and construction staking to assure installation of all project components including roads, parking areas, camp spurs, camp units, tent pads, paths, culverts, buildings, and utilities in accordance with the Drawings and Specifications. Provide all labor, equipment, instruments, materials, transportation and other incidentals necessary to complete the survey and construction staking in accordance with these specifications and acceptable engineering practice.
C. Provide record drawings of finished project.
1.4 PERMITS
A. Special permits for electrical work or any other permits required by County, State, or Federal laws or regulations shall be obtained by the Contractor.

WORK RESTRICTIONS
PART 1-GENERAL
1.1 USE OF PREMISES
A. Use of Site: Limit use of premises to work in areas indicated. Do not disturb por-tions of Project site beyond areas in which the Work is indicated.
1. Limits: Confine constructions operations to immediate area of work
2. Government Access: Allow for Government access to Project site. Main-tain areas for access and use by public visitors as directed by Contracting Officer (CO).
3. The campground site will be closed to public during construction.
B. Maintain access to existing highway parking, trails, walkways, lake corridors, and other adjacent used facilities. Do not close or obstruct theses accesses, or other occupied or used facilities without written permission from Government and au-thorities having jurisdiction.
1.2 WORK RESTRICTIONS
A. Nonsmoking Building: Smoking is not permitted within the buildings or within 25 feet (8 m) of entrances, operable windows, or outdoor air intakes.
B. Sanitary Facilities: Use of on-site Government sanitary facilities will not be per-mitted. Contractor shall provide temporary sanitary facilities for the duration of the project.
C. Overnight camping on the Site will be permitted, subject to the conditions of the Ranger District. Secure a permit from the USFS.
1.3 CONTRACTOR’S RESPONSIBILITIES
A. Contractor is responsible for all site safety and security.

B. The CO will designate areas on the site for material storage, stockpiling, and tem-porary storage buildings. Confine storage of materials to areas as approved by the
CO.
C. Provide adequate signing and barricades and take necessary safety measures to protect the public during all construction operations. Minimize disturbance of all undisturbed areas.
D. Submit project record drawings prior to project closeout and final payment.

SECTION 011500
HAZARDOUS MATERIALS REQUIREMENTS
PART 1 - GENERAL
1.1 SUMMARY
A. This Section includes general administrative requirements and procedures in the event hazardous materials are discovered during the project.
1.2 SUBMITTALS
A. Material Safety Data Sheets (MSDS): Retain copies on site of all material safety data sheets for all applicable products, including but not limited to: paints, adhesives, mastics, solvents and finishes, etc. used during the construction work.
Furnish copies of all MSDS’s to the Government upon request and include in the Project Record Document submittal.
B. Certification: Submit written certification that no asbestos-containing materials or hazardous materials have been used or incorporated in the new Work. Submit certification prior to Final Completion.
1.3 QUALITY ASSURANCE
A. No known asbestos containing materials are located in the buildings or on-site.
Contractor is responsible for reviewing the existing construction documents of the original building at the project site and becoming familiar with any hazardous materials in the work areas.
1.4 PROJECT/SITE CONDITIONS
A. If hazardous materials are discovered during the construction period; immediately notify the Contracting Officer. Abatement measures will be taken by the Government under separate contract. The Contractor shall coordinate sequencing and scheduling with that of the hazardous materials abatement work.
B. Hazard Communication Requirements: Comply with mandatory federal rules and regulations concerning Hazardous Communication, specifically for those regulations contained in 29 CFR 1910.1200 Hazard Communication.
1. Contractor working at sites under the control of the Government shall make available to the CO, upon request, copies of the Hazard Communi-cation Program used by their firm. In addition to this requirement, all reg-ulations to Multi-employer workplaces shall be adhered to. These regula-tions are found in 29 CFR 1910.1200, (e) (2) (i) through (e) (4) specifi-cally:

a. (e) (2) Multi-employer workplaces. Employers who produce, use or store hazardous chemicals at a workplace in such a way that em-ployees of other employer(s) may be exposed (for example, em-ployees of a construction contractor working on a site) shall addi-tionally ensure that the hazard communication programs developed and implemented under paragraph (e) include the following:
1) (3) (2) (i) The methods the employer will use to provide the other employer(s) with a copy of the material safety data sheet, or to make it available at a central location in the workplace, for each hazardous chemical the other em-ployer(s)’ employees may be exposed to while working;
2) (e) (2) (ii) The methods the employer will use to inform the other employer(s) of any precautionary measures that need to be taken to protect employees during the work-place’s normal operating conditions and in foreseeable emergencies; and,
3) (e) (2) (iii) The methods the employer will use to inform the other employer(s) of the labeling system used in the workplace.
b. (e) (3) the employer may rely on an existing hazard communica-tion program to comply with these requirements, provided that it meets the criteria established in this paragraph (e)
c. (e) (4) The employer shall make written hazard communication program available, upon request, to employees, their designated representatives, the Assistant Secretary and the Director, in accord-ance with the requirements of 29 CFR 1910.1200 (e).
2. The referenced regulations were excerpted from 29 CFR 1910.1200. This excerpt shall not be relied upon for compliance with mandatory federal, state and local regulations. Comply with all such regulations and shall be solely liable for insuring that all requirements under applicable regulations are met.

3.1 EXAMINATION:

A. Stay alert to the possibility that the work may uncover asbestos base containing materials, lead based paint, or other hazardous materials. If suspected materials are found, notify the CO and stop all work in the area immediately. If the sus-pected materials prove to contain asbestos, lead based paint or other hazardous materials the Government will arrange to have the materials abated in a timely manner.

KOOTENAI NF ACCIDENT PREVENTION
SECTION 011700
ACCIDENT PREVENTION
1.1 DESCRIPTION
A. The work of this section consists of establishing an effective accident prevention plan and providing a safe environment for personnel and visitors.
B. Contractor is responsible for all site safety. All construction methods and procedures shall be in compliance with the State of Montana, Department of Labor, Division of Labor Standards and Safety. Contractor is responsible for complying with current OSHA rules and regulations at all times during the term of this Contract.
1.2 SUBMITTALS
A. Accident Prevention Plan: Before on-site work begins, submit a company approved accident prevention plan. This plan will be posted in the contract file. Design the plan to address Federal, State, and Local Occupational Safety and Health requirements that apply to this project. No progress payments will be processed until the plan is received. As a minimum the plan shall include:
1. Name, position title and contact information of company executive responsible for approving the Accident Prevention Plan.
2. Name and contact information of supervisor responsible to carry out the plan.
3. Outline of each phase of the work, the hazards associated with each major phase, and the methods proposed to ensure property protection and safety of the public, government personnel, and the Contractor's employees. Identify the work included under each phase by reference to specification section or division numbers.
4. Contingency plans for emergency situations such as medical, fire, hazard material spills and other contract assessed hazard prevention and abatement requirement needs that apply to this project.
B. Certificates: Provide certificates from a mechanic that all mechanical equipment has been inspected and meets OSHA requirements.
C. Submit a copy of test reports, as required by OSHA, for personnel working with hazardous materials.

D. Submit a brief report of safety meetings and of inspections.
E. Upon request, submit proof of employees' qualifications to perform assigned duties in a safe manner.
1.3 QUALITY ASSURANCE
A. Clauses entitled "Accident Prevention" and "Permits and Responsibilities" of the General Provisions: In case of conflicts between Federal, state, and local safety and health requirements, the most stringent shall apply. Equipment or tools not meeting OSHA requirements will not be allowed on the project sites. Failure to comply with the requirements of this section and related sections may result in suspension of work.
B. Qualifications of Employees:
1. Ensure employees are physically qualified to perform assigned duties in a safe manner.
2. To as great an extent as possible, do not allow employees to work whose ability or alertness is impaired because of drugs, fatigue, illness, intoxication, or other conditions that may expose themselves or others to injury.
3. Operators of vehicles, mobile equipment, hoisting equipment, and hazardous plant equipment shall be able to understand signs, signals, and operating instructions, and be capable of operating such equipment. Provide operating instructions for all equipment. Individually test newly hired operators by an experienced operator or supervisor to determine if they are capable of safely operating equipment.
1.4 ACCIDENT REPORTING
A. Reportable Accidents: A reportable accident is defined as death, occupational disease, traumatic injury to employees or the public, property damage by accident in excess of $100, and fires. Within 7 calendar days of a reportable accident, fill out and forward to the CO a CA-1 form, which may be obtained from CO.
B. All Other Accidents: Report all other accidents to the CO as soon as possible and assist the CO and other officials as required in the investigation of the accident.
PART 2 – PRODUCTS
2.1 FIRST AID FACILITIES
A. Provide adequate facilities for the number of employees and the type of construction at the site.
2.2 PERSONNEL PROTECTIVE EQUIPMENT

A. Meet requirements of NIOSH and MSHA, where applicable, as well as ANSI.
PART 3 – EXECUTION
3.1 EMERGENCY INSTRUCTIONS
A. Post telephone numbers and reporting instructions for ambulance, physician, hospital, fire department, and police in conspicuous locations at the work site.
3.2 ESCAPE ROUTES
A. Provide and maintain adequate escape routes at all times in accordance with the Life Safety Code (NFPA 101-91). No corridor, aisle, stairway, door, or exit shall be obstructed or used in a manner that interferes with escape routes.
3.3 PROTECTIVE EQUIPMENT
A. Inspect personal protective equipment daily and maintain in a serviceable condition.
Clean, sanitize, and repair, as appropriate, personal items before issuing them to another individual.
B. Inspect and maintain other protective equipment and devices before use and on a periodic basis to ensure safe operation.
3.4 SAFETY MEETINGS
A. As a minimum, conduct weekly 15-minute "toolbox" safety meetings. These meetings shall be conducted by a foreman and attended by all construction personnel at the worksite.
B. Conduct monthly safety meetings for all levels of supervision. Notify the CO so the CO may attend. Use these meetings to review the effectiveness of the Contractor's safety effort, to resolve current health and safety problems, to provide a forum for planning safe construction activities, and for updating the accident prevention plan.
The CO will enter the results of the meetings into the government’s daily log.
3.5 HARD HATS AND PROTECTIVE EQUIPMENT AREAS
A. Designate a hard hat area. Post the hard hat area in a manner satisfactory to the CO.
B. It is the Contractor's responsibility to require all those working on or visiting the site to wear hard hats and other necessary protective equipment at all times. As a minimum, provide two hard hats for use by visitors. Change liners before reissuing hats.
3.6 TRAINING
A. First Aid: Provide adequate training to ensure prompt and efficient first aid.

B. Hazardous Material: Train and instruct each employee exposed to hazardous material in safe and approved methods of handling and storage. Hazardous materials are defined as explosive, flammable, poisonous, corrosive, oxidizing, irritating, or otherwise harmful substances that could cause death or injury.

SECTION 012000
MEASUREMENT AND PAYMENT
A. The intent of this section is to explain, in general; what is and what is not included in a contract item; the limits or cut-off points where one item ends, and another begins; and method of measurements and basis of payment for work items listed in the Schedule of Items.
B. Work: Furnishing all labor, materials, equipment, and other incidentals necessary to successfully complete the project or any portion of it and carrying out all duties and obligations imposed by the contract on the Contractor.
C. Payment: For each individual item listed here and in the Schedule of Items, payment shall be full compensation for all work related to the particular item in accordance with these specifications and as shown on the drawings.
D. Measurement and payment for contract work shall be made only for and under those pay items included in the Schedule of Items. All other work and materials shall be considered incidental or as included in the payment for items shown.
1.2 DETERMINATION OF QUANTITIES
A. The following measurements and calculations shall be used to determine contract quantities for payment.
1. For individual construction items, longitudinal and lateral measurements for area computations shall be made horizontally or corrected to horizontal measurement unless otherwise specified. Measurements for geotextiles, netting and erosion control blankets shall be along slope lines.
2. For excavation or embankment volumes, the average end area method shall be used to compute volumes. However, if in the judgment of the Contracting Officer (CO), the average end area method is impractical, measurement shall be made by volume in hauling vehicles or by other three-dimensional methods.
3. For Structures, they shall be measured according to neat lines shown on the drawings or as altered by the CO, in writing, to fit field conditions.

4. For items that are measured by the linear foot, such as pipe culverts, fencing, guardrail, piping, utilities, and underdrains, measurements shall be made parallel to the base or foundation upon which the structures are placed.
5. For aggregates weighed for payment, the tonnage shall not be adjusted for moisture content, unless otherwise provided for.
6. For standard manufactured items (such as fence, wire, plates, rolled shapes, pipe conduits) identified by gauge, weight, section dimensions, and so forth, such identifications shall be considered the nominal weights or dimensions.
Unless controlled by tolerances in cited specifications, manufacturer's tolerances shall be accepted.
1.3 UNITS OF MEASUREMENT
A. Payment shall be by units defined and determined according to U.S. Standard measure and by the following:
1. Acre: Make longitudinal and transverse measurements for area computations horizontally.
2. 50lb Bag: Measurement will be for the actual number of 50lb bags of standard bentonite grout.
3. 94lb Bag: Measurement will be for the actual number of 94lb bags of standard cement or grout.
4. Cubic Yard (CY): A measurement computed by one of the following methods:
a. Excavation, Embankment, or Borrow. The measurement computed by the average end area method from measurements made longitudinally along a centerline or reference line.
b. Material in Place or Stockpile. The measurement computed using the dimensions of the in-place material.
c. Material in the Delivery Vehicle. The measurement computed using measurements of material in the hauling vehicles at the point of delivery. Vehicles shall be loaded to at least their water level capacity. Leveling of the loads may be required when vehicles arrive at the delivery point.
5. Each (EA): One complete unit, which may consist of one or more parts.
6. Gallons (GAL): The quantity shall be measured by any of the following methods:
a. Measured volume in container.
b. Metered volume by approved metering system.
c. Commercially package volume.
7. Hour (HR): Measurement will be for the actual number of hours (or fraction thereof) ordered by the Contracting Officer and performed by the contractor.

8. Linear Foot (LF): Measurement of work along its length from point-to-point; parallel to the base or foundation. Do not measure overlaps.
9. Lump Sum (LS): Do not measure directly. The bid amount is complete payment for all work described in the contract and necessary to complete the work for that item.
10. Mile: Measured horizontally along the centerline of each roadway, approach, or ramp.
11. Pound (LB): For sacked or packaged material, measurement will be the net weight as packed by the manufacturer.
12. Square Foot (SF): Measured on a plane parallel to the surface being measured.
13. Square Yard (SY): Measured on a plane parallel to the surface being measured.
14. Ton: Measured as a short ton consisting of 2,000 pounds.
PART 2 - METHOD OF MEASUREMENT
2.1 GENERAL
A. One of the following methods of measurement for determining final payment is designated on the Schedule of Items for each pay item:
1. LUMP SUM QUANTITIES (LSQ). These quantities denote one complete unit of work as required by or described in the contract, including necessary materials, equipment, and labor to complete the job. They shall be measured complete and in-place.
2. ACTUAL QUANTITIES (AQ) - These quantities are determined from actual measurements of completed work.
3. DESIGNED QUANTITIES (DQ) - These quantities denote the final number or units to be paid for under the terms of the contract. They are based upon the original design data available prior to advertising the project. Original design data include the preliminary survey information, design assumptions, calculations, drawings, and the presentation in the contract. Changes in the number of units shown in the Schedule of Items may be authorized under any of the following conditions:
• As a result of changes in the work authorized by the CO.
• As a result of the CO determining that errors exist in the original design that cause a pay item quantity to change by 15 percent or more.
• As a result of the Contractor submitting to the CO a written request showing evidence of errors in the original design that cause a pay item quantity to change by 15 percent or more. The evidence must be verifiable and consist of calculations, drawings, or other data that show how the designed quantity is believed to be in error.

4. STAKED QUANTITIES (SQ) - These quantities are determined from staked measurements prior to construction.
5. VEHICLE QUANTITIES (VQ) - These quantities are measured or weighed in hauling vehicles.
PART 3 – DEFINITION OF CONTRACT ITEMS
3.1 SCHEDULE OF ITEMS
A. The project Schedule of Items includes several sections to organize the contract items into Base (Bid) Items and Additive Alternates (Optional Items). When a pay item is described as a Base Item, the description will be the same when that item or a similar item is included as an Optional Item. Provide item pricing for all items listed in the Schedule of Items. Contract award will be made for all Base Items. Those items listed as Optional Items will be awarded based on available project funding. The Optional Items are not listed in priority order and may be awarded in any combination of individual items.
B. No item that is required by the Contract Documents for the proper and successful completion of the work will be paid for outside of or in addition to the prices submitted in the Bid Proposal. All work not specifically set forth as a pay item in the Bid Proposal shall be considered a subsidiary obligation of the Contractor and all costs in connection therewith shall be included in the prices bid.
3.2 DEFINITION OF BASE BID ITEMS
A. Base Bid Item #1: Mobilization.
1. General: This bid item shall include mobilization, bonding, insurance and submittals; all materials, equipment, labor and incidentals required to perform all operations necessary for the movement of personnel, equipment, supplies and incidentals to the project site, and for all other work and operations that must be performed or that cause costs to be incurred prior to beginning work on the various items on the project site.
2. Work Included:
• All labor, tools, equipment, materials, and incidentals necessary to complete the work as shown and specified;
• Transport and set up all equipment, materials and other items needed to complete the project;
• Site safety signage, access coordination, site security, traffic control, staging area preparation and reclamation;
• Bonding and Insurance and construction permitting;

3. Measurement: Measurement for mobilization shall be one lump sum (LS) item.
4. Payment: Payment shall be by the lump sum (LS) item listed on the Proposal. Payment of this bid item will be allowed once the Contractor is fully mobilized, all submittals are received, and bond and insurance certificates have been submitted and approved; thereon 100% payment will be allowed (subject to 5% withholding). This bid item may not exceed five percent (5%) of the total base bid.
B. Base Bid Item #2: Sewer Pressure Pipe 1.5” (GSP & HDPE)
1. General: This bid item includes installation of 1.5” GSP and HDPE sewer piping and appurtenances shown on the plan and profile sheets and as detailed in the general detail sheets of the Construction Drawings.

UTILITY COMPANY COORDINATION
A. The work of this section consists of coordination with utility companies for connection to their systems. For the Rexford project 3-phase power needs to be routed to the site to replace the existing buried single phase power. This work was preliminarily coordinated with the local power supplier Lincoln Electric Coop. The Contractor is responsible for coordinating and paying for the buried 3 phase power through Lincoln Electric.

1.2 COORDINATION
A. Electrical: Coordinate electrical utility work with Lincoln Electric Cooperative, 406-889-3301.
B. Where work by a utility company is required in conjunction with construction, such as installation of cable by a utility company in a common trench with other utilities to be installed by the Contractor, the Contractor is solely responsible for contacting the utility company to coordinate any and all work required by the utility provider.
1.3 PAYMENT TO UTILITY COMPANIES
A. Payment to Utility Companies:
1. Where work is to be performed by the utility company that requires payment to the utility company, payment will be made by the Contractor. Include an allowance for Lincoln Electric Coop to install the 3-phase power to the site. See attached correspondence with Lincoln Electric Coop.
2. Where the Contractor identifies the need for work by the utility company that requires payment and no known contractual arrangement by Government and utility company has been made in advance, the Contractor shall immediately notify the Contracting Officer.

1.2 GENERAL PROJECT COORDINATION
A. Coordination of Trades: Coordinate construction operations included in the various sections of the Specifications to provide an efficient and orderly installation of each part of the Work. Coordinate construction operations included under different sections of the Specifications that depend on each other for proper installation, connection or operation.
1. Schedule construction operations in the sequence required to obtain the best results where the installation of one part of the Work depends on installation of other components before or after that part.
2. Coordinate installation of different components to provide maximum accessibility for required maintenance, service, testing and repair.
3. Accommodate items scheduled for later installation.
B. Notification: Where necessary, prepare and distribute memoranda to each party involved, outlining special procedures required for coordination. When applicable, include notices, reports and meeting minutes as part of the memoranda.
