About this file
This questionnaire is a consultant profile and evaluation form issued by the Florida Department of Financial Services, Office of Insurance Regulation, for vendors seeking to conduct market conduct examinations of insurance entities, specifically for a multi-state examination of the National Council on Compensation Insurance, Inc. The document requests comprehensive information regarding firm capabilities, qualifications, and resources necessary to perform full-scope and targeted market conduct examinations. Respondents must demonstrate compliance with National Association of Insurance Commissioners (NAIC) Market Regulation Handbook standards and provide detailed documentation of examination experience, including the number and types of market conduct examinations performed over specified timeframes, with particular emphasis on workers' compensation insurance and advisory/statistical organization examinations conducted for state insurance departments.
The questionnaire establishes mandatory criteria and professional qualification requirements, including verification that examiners hold appropriate certifications such as Certified Insurance Examiner (CIE) or Accredited Insurance Examiner (AIE) designations as defined by the NAIC Market Regulation Handbook. Respondents must disclose standard hourly rates for Examiners-in-Charge and Senior Examiners, provide proof of Florida Department of State registration or exemption, certify drug-free workplace compliance, and complete E-Verify authorization. Additional requirements include disclosure of any lawsuits, regulatory suspensions, conflicts of interest involving family members employed by insurers, and employment history with the Florida Office of Insurance Regulation within the past two years. The document also contains an Identical Tie Response Form allowing vendors to qualify for preference consideration based on minority ownership, veteran status, Florida business domicile, or drug-free workplace certification, with supporting documentation requirements including resumes, NAIC training completion certificates, and relevant examination experience documentation.
