Quality Assurance Surveillance Plan Summary

This is a Quality Assurance Surveillance Plan (QASP) for a Land Mobile Radio (LMR) On-Site Technologist contract. The contract is Firm-Fixed-Price (FFP) and requires the contractor to provide non-personal services including all personnel, equipment, supplies, transportation, tools, materials, supervision, and other necessary items to perform services as outlined in the Performance Work Statement (PWS). The contractor must ensure services are performed in accordance with PWS standards and maintain a quality control program per FAR Part 46 to identify, prevent, and ensure non-recurrence of defective services.

Government surveillance will be conducted using four methods: 100% Inspection, Random Sampling, Periodic Inspection, and Customer Feedback. The QASP establishes four performance objectives with specific Acceptable Quality Levels (ALS): (1) On-hours LMR system failure response within 10 minutes with 100% compliance; (2) Off-hours on-call technician response within 2 hours with 100% compliance; (3) Weekly status reports submitted by close of business Monday with 95% on-time submission; and (4) LMR System Inventory Reports submitted initially within 30 calendar days and updates within 10 working days upon request, both with 100% accuracy. Nonconformances are classified as Level 1 (Minor—promptly correctable), Level 2 (Major—requiring root cause analysis and Corrective Action Plans), or Level 3 (Critical—representing serious internal control weaknesses). The Contracting Officer's Representative (COR) will maintain surveillance schedules and checklists in the Procurement Integrated Enterprise Environment (PIEE), submit monthly COR Status Reports, verify contractor manpower reporting to SAM.gov annually, and use documented surveillance data to support CPARS ratings. No deduct plan is included in this acquisition.

The Contractor shall ensure services are performed in accordance with the PWS and shall identify, prevent, and ensure non-recurrence of defective services.

4.2. Government Quality Assurance. The Government shall evaluate the Contractor’s performance under this contract in accordance with this QASP. This plan is primarily focused on what the Government must do to ensure that the Contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and acceptable quality levels.

5. Government Surveillance

5.1. Methods of Surveillance. The following Methods of Surveillance will be utilized: 100% Inspection, Random Sampling, Periodic Inspection, and Customer Feedback. Complaints will be investigated by the COR. Validated complaints that are contractual non-conformances will follow the procedures in paragraph 6.

All documentation, including the COR Status Report and any Nonconformance Reports, will be filed in PIEE no later than the 15th of the following month.

6. Nonconformance Reports (NCRs)

Nonconformance occurs when a Contractor fails to meet contract terms. All instances will be documented via a Nonconformance Report Worksheet. Nonconformances are classified as Level 1 (Minor), Level 2 (Major), or Level 3 (Critical).

6.1. Level 1 (Minor). Nonconformances that can be promptly corrected and do not require root cause analysis or preventive action.

6.3. Level 3 (Critical). Nonconformances that represent a serious weakness in internal controls with significant impact on performance. These may lead to contract remedies and are issued after repetitive Level 2 failures or for hazardous conditions.

7. Deduct Plan

This acquisition does not include a Deduct Plan.

8. Data Analysis

Data analysis will be performed to identify trends in cost, schedule, or performance. Results will be documented in COR Status Reports and may be used to adjust surveillance levels.

9. Acceptance of Services

When all services have been deemed acceptable, the COR will accept the services and authorize payment by approving the Contractor’s Receiving Reports in PIEE/WAWF.

10. COR Status Report

COR Status Reports will be submitted monthly via PIEE, providing a synopsis of the Contractor's performance, surveillances performed, and any nonconformances.

11. Contractor Manpower Reporting
