About this file

This document is a Quality Assurance Surveillance Plan (QASP) for Confined Space Entry Training, designed to manage and assess contractor performance across three key areas: Timeliness, Course Quality, and Business Relations. The QASP requires the contractor to provide timely reporting, including a Training Completion Certificate for each student, a comprehensive course report within 10 days of course completion, and a monthly summary of training activities to the Contracting Officer's Representative (COR). Course quality standards mandate logically organized student materials, with 100% of students receiving all necessary resources to successfully complete the training, and an expectation that all course materials, curriculum, references, and syllabi meet specified requirements for effective learning.

Quality Assurance Surveillance Plan (QASP) for Confined Space Entry Training

The QASP is used for managing contractor performance assessment by ensuring that systematic quality assurance methods validate that contractor quality control efforts are timely, effective, and are delivering the results specified in the contract or task order. The QASP will cover the 3 following areas:

Timeliness: Provided reports and other output to the COR 100% on time. Respond to communication inquiries within 2 business days, 95% of the time.

Within the Performance Work Statement (PWS) there are various deliverables that have different timelines, including the following:

• Each student shall be provided with a Training Completion Certificate upon successfully completing the course.

• The contractor shall provide to the COR, within 10 days of the successful completion of a course, with a report containing, at a minimum, a signed student roster, copies of student completion certificates, student critiques, and instructor comments and observations.

• Once every 30 calendar days, the Contractor shall provide the COR with a summary of the completion status of each task identified in Section 3. This will include a listing of classes given to date, the number of students trained to date, number of students successfully passing the course to date, and locations and dates of upcoming classes.

Course Quality: Student course material is organized logically and easily understandable.

100% of students attending are provided with all materials and instructions to successfully pass the course.

Within the PWS there are requirements for the curriculum, references used, syllabus, course material, course critiques, and other details to ensure students are provided with everything necessary to be able to successfully complete the course.

Business Relations: Issues and problems are resolved in a timely, professional manner.

All questions, concerns, or issues concerning performance of duties within the PWS are to be brought to the attention of the Contracting Officer (KO) or the Contracting Officer’s Representative (COR) as soon as possible.
