About this file

This is a Past Performance Questionnaire (PPQ) template issued by the 60th Contracting Squadron at Travis Air Force Base, California, to collect customer references for contractors bidding on Elevated Work Platform Systems Maintenance or Repair services. The questionnaire must be completed by past or present customers of the competing contractor and submitted to Contract Specialist Clark Teninty at clark.teninty.1@us.af.mil by 4 March 2026 at 4:30 PM PDT. Responses will become part of the official source selection record and can be confirmed by contacting Teninty at 707-424-7737.

The questionnaire requires respondents to provide information about both the offeror (contracting company) and the respondent organization, followed by detailed contract information including contract name/number, description, performance period, and contract values. Respondents must rate the contractor's performance on a 1-5 scale across six categories: Quality of Service, On-Time Delivery of Service, Management of Service, Communication, and Contract Terms and Condition Adherence, with an overall performance rating. Ratings of 1 (Unsatisfactory), 2 (Marginal), or 5 (Exceptional) require written explanations. The questionnaire also asks whether the contractor received any Corrective Actions Reports or similar deficiency notices and whether the respondent would award another contract to the contractor.
