About this file
This is a Past Performance Questionnaire (PPQ) template designed for evaluating contractor performance on the solicitation for renovating Wards 2K and 2B for Optometry/Ophthalmology Clinical Use (Solicitation 36C24726R0134) at the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 7. The questionnaire serves as an evaluation instrument to be completed by previous clients of offerors submitting proposals.
The PPQ template requires offerors to provide organizational information including the contractor name, reference client information, contract number, project description, contract/task order amount, performance period, and contractor role (prime or subcontractor). Evaluators must rate contractor performance across thirteen performance categories using a five-point numerical scale: 5 for Superior Performance, 4 for Very Good Performance, 3 for Satisfactory Performance, 2 for Marginal Performance, and 1 for Unacceptable Performance, with N/A for not applicable ratings. The evaluation criteria assess compliance with contract requirements, on-site management effectiveness, personnel appropriateness, timeliness of documentation submissions, construction schedule adherence, safety responsiveness, subcontractor coordination, customer and government personnel relations, problem-solving and change negotiation capabilities, financial difficulty management, responsiveness to letters of concern and cure notices, warranty response, and overall project performance. Evaluators must also indicate whether they would recommend or work with the contractor again and provide contact information and additional comments explaining any low ratings. Completed questionnaires must be returned directly by the reference clients to cayley.blommers@va.gov, with offerors bearing responsibility for ensuring accurate and verifiable point of contact information.
