About this file

This document is a Past Performance Questionnaire from the 28th Contracting Squadron Group of the U.S. Air Force at Ellsworth Air Force Base. The form is designed to comprehensively evaluate a contractor's performance across multiple dimensions, including personnel quality, contract pricing, resource management, customer responsiveness, business relationships, safety compliance, problem resolution, mission adaptability, and quality control. The questionnaire allows the evaluator to rate the contractor's performance on a scale of Acceptable (A), Unacceptable (U), or Unknown (UNK) across nine specific performance criteria, with space for detailed remarks. Additional questions include whether the contractor was issued any cure or show cause notices, contract termination status, and whether the agency would award another contract to the contractor.
