PAST PERFORMANCE QUESTIONAIRE
Instructions. Provide the information requested in this form for each contract/project being used for past performance data. Provide frank, concise comments regarding the contractor’s performance on the work performed. Provide a separate completed form for each contract/project submitted. Limit the length of the Past Performance Questionnaire (PPQ) to the length set forth in FAR 52.212-1, section L, of the solicitation. (2 Pages per submission). All PPQs must be submitted to Amn Tyler Menster at tyler.menster@us.af.mil and TSgt Josh Johnson at joshua.johnson.164@us.af.mil.
