Addendum to 52.212-1- Instructions to Offerors-Commercial Products and Commercial Services (Sep 2023)

PHYSICALLY REMOVED FROM ANY RESULTANT AWARD***

Addendum to 52.212-l(a), NAICS code and small business size standard. The NAICS code is 517112 and the size standard is 1,500 employees for this requirement.

Addendum to 52.212-1 (b), Submission of Offers. Submit signed and dated offers in accordance with block 8 of the SF-1449 and any amendments.

General Instructions.

A. To assure timely and equitable evaluation of the offer, the offeror must follow the instructions contained herein. Offerors are required to meet all solicitation requirements.

Failure to meet a requirement may result in an offer being ineligible for award.

B. The offer shall include two separate parts:

Part I – Technical Proposal Part II – Cost/Price Proposal

C. Complete the necessary fill-ins certifications in provisions and in the on-line Representations and Certifications at Sam.gov. Return the provision FAR 52.212-3, Offeror Representations and Certifications - Commercial Items along with the offer. For other provisions and clauses in the solicitation, the offeror is required to submit the pages that require an offeror fill-in.

F. Questions must be submitted via e-mail to the personnel noted above. All questions are due Thursday 31 July 2025 at 11:00 AM CST. All questions and answers will be posted to SAM.gov.

G. Offer Submission Instructions: Offers must be submitted electronically via e-mail to the personnel noted above. All documents submitted must be saved in .pdf format and shall follow the instructions in FAR 52.212-1. All quotations are due 15 August 2025 at

1:00 PM CST.

Late offers will be processed in accordance with FAR 52.212-l(f), "Late submission, modifications, revisions, and withdrawals of offers."

Offers shall be in 8 1/2" X 11" page. A page is defined as one face of a sheet of paper containing information. The text size shall be no less than 12 pitch. Offers shall use at least 1-inch margins on the top and bottom and 1-inch side margins.

H. Specific Instructions

Part 1 - Technical Submission: Limited to six (6) pages total.

The technical Part shall be comprised of two (2) parts, each being limited to three (3) pages.

1. Your responses will be evaluated against the Technical Factor as defined in Addendum 52.212-2. Quotes shall not simply rephrase or restate the Statement of Objectives (SOO) or contain general statements such the technical portion of the offer meets solicitation requirements. Offerors shall assume the Government has no prior knowledge of the offeror's experience. The Government will base its evaluation on the information presented in the offer.

2. Technical Plan. Submit a written narrative which demonstrates an understanding of the project and responsibilities as outlined in the Statement of Objectives, not to exceed three

(3) pages in length. The offeror shall include sufficient documentation describing the performance plan to allow through evaluation of how services will be provided in accordance with SOO.

3. Resume. Offerors are required to provide a Resume with a minimum of 3 years’ experience in the application, installation and testing of the specified systems and equipment.

Note: Although this element will not be evaluated, its omission will render the offer ineligible for award consideration.

Part 2 – Price Submission

1. Complete blocks 12, 17a, and 30a, band c of the Standard Form (SF) 1449. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation, with attachments. The solicitation constitutes the model contract.

52.212-2 Evaluation—Commercial Products and Commercial Services.

As prescribed in 12.301(c), the Contracting Officer may insert a provision substantially as follows:

Evaluation—Commercial Products and Commercial Services (Nov 2021)

1. Evaluation Factors for Award.

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

2. Synopsis of Evaluation

Step One: Evaluate all quotations for responsiveness defined as meeting the submitting requirements in Addendum to 52.212-1- Instructions to Offerors-Commercial Products and Commercial Services (Sep 2023).

Step Two: Evaluate prices and order all responsive quotes based upon the quoter's Total Evaluated Price (TEP) in ascending order form lowest to highest price.

Step Three: The lowest priced responsive quote shall be evaluated for technical acceptability. In the event that the lowest quote does not yield an acceptable technical rating, the evaluation process will continue until the lowest priced responsive quotes is found to have a technical rating. Once the first lowest priced responsive quote is found, no further quotes will be evaluated for technical acceptability.

The Government will base its evaluation on the information presented in the quote.

In order to be technically acceptable, the quote must meet all the following criteria:

The quote shall be evaluated to determine whether the proposal is acceptable or unacceptable, using the ratings and descriptions outlined in the table below. One technical rating will be assigned to each technical sub factor. In order to be considered awardable, there must be an "acceptable" rating in every sub factor.

Step Four: Pursuant to FAR 12.602, contract award will be made using Simplified Acquisition Procedures in accordance with FAR 13.106. The government shall make an award to the lowest price technically acceptable offeror.
