About this file
This is a Work Summary & Progress Report form for the National Shrine Project's Flat Marker Raise and Realignment contract work. The form serves as a reporting document for contractors to document completed work and submit it for government acceptance, rather than as an invoice.
The form captures progress on eight distinct work categories applicable to Options 2, 3, 4, and 5 of the contract: (1) Compaction Testing, (2) Flat Granite Marker Setting, Alignment & Backfill onto Headstone Support System, (3) Soil Compaction Testing, (4) Base Compaction Testing, (5) Repair & Replacement (R&R) Flat Marker Headstones, (6) Clean Flat Markers, (7) Agricultural Soil Testing and Recommendations, and (8) Turf Renovation. For each work category, the contractor reports the total quantity of units completed (measured in each/EA or square feet) and the corresponding cemetery section(s) where work was performed. The form includes an Inspection/Acceptance clause reserving the Government's right to inspect, test, and require re-performance of nonconforming services at no additional cost. Contractors must report any circumstances adversely affecting work progress, such as weather, turf conditions, strikes, or government delays. Both the contractor and the Cemetery Director/Contracting Officer Representative (COR) must sign and date the form to confirm completion and acknowledge receipt. The form is designed to be duplicated as necessary for multiple reporting periods.
