STATEMENT OF WORK

Multi-phase Door Replacement 2025 Pine Ridge Hospital

Pine Ridge, SD

1. Project Description A. Title: Multi-phase Door Replacement 2025

B. Location: This project is at the Pine Ridge Hospital, in Pine Ridge, SD.

C. Period of Performance: 270 calendar days from Notice to Proceed.

D. Description: The project defined within this Scope of Work is to replace/upgrade 67 doors and/or door components throughout the hospital. Most of the door and door frames from the original hospital construction are worn and/or damaged. Many of the high traffic doors will be upgraded to stainless steel wrapping to prevent excessive wear. Contractor shall install products following all manufacturers’ specifications. All doors replaced must follow NFPA 101 life safety code on doors and door openings and follow the facilities Life Safety Map. For reference, the Facility Life Safety Map can be seen in Appendix D.

E. Background: The existing doors in the Pine Ridge hospital divide rooms and corridors used for all functions that occur within a hospital. Daily operations including moving patients, housekeeping carts and heavy medical equipment with high frequency reduce the lifespan of the doors.

2. Scope of Work (SOW): The scope of this project includes all demo, new installation, and clean up for installation of the products described below. All electrical installation shall be done by an electrician licensed in the state of South Dakota. The location of all doors to be worked on are outlined in Appendix B construction drawings. The change to each door and requirements of each door are indicated with notes within Appendix B. Only one door at a time is to be worked on to minimize any disruptions to the hospital at each location. No work is to be started until all components of each door assembly are onsite and ready to be installed. All doors must follow NFPA 101 Life Safety code for self-closing, self-latching (positive latching) and fire rating requirements as well as all other door requirements. If the contractor is unsure of the requirements of doors/hardware, contact the COR for clarification.

The contractor shall provide all engineering, design, material, labor, equipment and supervision required to accomplish the following:

A. Demolition: Each existing door and door hardware to be replaced as indicated in Appendix B shall be removed and returned to the facility. Upon the facility’s discretion, the demolished doors and hardware may be disposed of properly.

B. Warranty: Provide a 1-year labor and material warranty at no additional cost to the government which provides for the repair of all work and equipment that was provided under this scope of work.

C. Electrical: The contractor shall follow manufacturer’s instructions on power requirements and installation for each powered door to be replaced as indicated in Drawings. The powered/automatic mechanisms must follow NFPA 101 Life Safety Code and be UL listed hardware.

D. Finishes: The contractor shall ensure that the finish of replaced door frames match existing. If an exterior door is to be replaced, the exterior frame must have its silicone caulking replaced. The caulking must be paintable so the facility can paint to match new facility-wide painting scheme. All walls damaged through door/frame replacements must be repaired to match surrounding finishes.

E. Specialties: Doors to be replaced that have an existing keypad entry shall retain that keypad entry.

Doors with a self-latching requirement must have hardware that automatically latches the door in the closed position. See NFPA 101 on self-latching (positive latching). All doors must be solid-core bonded wood doors. Where fire rating is required, NFPA 101 requirements for fire rated doors must be followed including its reference to NFPA 80. All stainless-steel armor kickplates must be UL listed. If a fire rate door requires kickplate, ensure whole door assembly is fire rated and labeled, including glazing.

F. Openings: The attached Drawing (see Appendix B) gives location and door requirement of each door to be replaced, modified, or upgraded in this project. All doors involved in this project are to be replaced with a like dimensioned door. The contractor is to field verify dimensions of all doors. Each door that has

Statement of Work Page 5 a 1 ¾” wood core is to be replaced with a 1 ¾” wood core door wrapped in stainless steel unless otherwise noted. The contractor shall field verify all door dimensions and other details on all doors involved in this project and present it in their bid schedule of doors. The stainless-steel plating shall be installed at the factory and shall cover 3 sides of the door. If field installing armor-plating, verify with COR that the assembly will be UL listed. See Appendix C for an example of the armor plating. This appendix is only an example as an equivalent plating can be submitted. The doors installed in this project must be UL listed and labeled properly if fire rating is required. All doors must follow NFPA 101 standards for doors.

G. Clean Up: Jobsite shall be cleaned daily and must comply with the Infection Control & Safety Officer’s requirements given with the ICRA and PRCA. The construction areas involved shall be free of any debris, rubbish and leftover material scraps upon completion of the project. Trash receptacles used for the collection of debris and their removal from the project site shall be provided by the contractor. The site will be inspected by the engineer prior to final payment to ensure the sites return to their original condition prior to construction.

H. Site Restoration: The construction site shall be free of any materials, debris and/or rubbish. The contractor will be responsible for the removal of this debris and must restore the site to its original condition as approved by the project engineer. Photos are to be taken before construction begins for documentation of the site’s original condition by the contractor.

I. Addition Facility Requirements: Contractor shall comply with the requirements of the hospital’s Facility Manager, Infection Control Officer and Safety Officer. These requirements will be laid out during the pre-construction meeting and will include providing a separate negatively pressurized workspace, vented to return ducts, or exhausted to the exterior. Other permits and requirements will be given after the pre-construction meeting and the ICRA and PRCA assessments are completed.

CLIN OPTION: CLIN line items will be broken down by construction drawing page, see Appendix B. All door replacements/repairs on each page will be bid on separately and submitted as a CLIN line item, e.g., Drawing sheet A401 for Appendix B has 3 distinct doors for replacement or repair that will be lumped into one CLIN line item. This will give the following Table that lists how the CLIN numbers will line up with the work presented in this SOW. Appendix A, quote schedule, may also be useful in identifying which doors belong to each CLIN line.
