Quality Assurance Surveillance Plan (QASP)

1. Purpose. This Quality Assurance Surveillance Plan (QASP) provides a standard of surveillance for monitoring contractor execution of the training Curriculum and support for the Centrifuge-Based Flight Environment Training (CFET) contract. It provides the approach the Government will use to conduct surveillance over the performance aspects of the effort. The QASP is used by Government Quality Assurance personnel to ensure the standards of the contract are met. The Contracting Officer's Representative (COR) and the Course Curriculum Model Manager (CCMM) monitor tasks issued under this requirement.

This plan uses quality metrics to evaluate the services the contractor is required to perform. This surveillance assures the Government that the contractor's performance is acceptable.

Any noncompliance with the contract performance requirements is not acceptable. The QASP is based on the premise that the contractor, not the Government, is responsible for management and quality control actions to meet the terms of the contract.

2. Government Resources. The following definitions for Government resources are applicable to this plan:

Contracting Officer - A person duly appointed with the authority to enter into (Procuring Contracting Officer) (PCO)) or administer (Administrative Contracting Officer) (ACO)) contracts and make related determinations and findings on behalf of the Government. The PCO for this contract is GT27000. The ACO will be designated in the resulting contract.

Contracting Officer's Representative (COR) - An individual appointed in writing by the PCO to administer the contract.

NSTI Course Curriculum Model Manager (CCMM) - The CCMM is responsible for surveillance of the CFET course performance. The CCMM reports all surveillance findings to the COR. The Naval Survival Training Institute (NSTI) is the designated CCMM for all Naval Aviation Survival Training Program curricula, including the CFET course. NSTI is designated as the CCMM and will provide a training POC for this contract.

NMOTC Training Safety Officer (TSO) - The NMOTC TSO is responsible for surveillance of the CFET course safety. The NMOTC TSO reports all findings to the COR. NMOTC TSO is designated as the training safety POC for this contract.

3. Responsibilities. The following Government resources shall have responsibility for the implementation of this QASP:

Contracting Officer - The Contracting Officer ensures compliance with the terms of the contract, and safeguards the interests of the United States in the contractual relationship. It is the Contracting Officer that assures the Contractor receives impartial, fair and equitable treatment under the contract. The Contracting Officer is ultimately responsible for the final determination of the adequacy of the Contractor’s performance.

Contracting Officer's Representative (COR) - The COR is responsible for technical administration of the contract and assures proper Government surveillance of the contractor’s performance. The COR is not empowered to make any contractual commitments or to authorize any changes on the Government’s behalf. Any changes that the contractor deems may affect contract price, terms or conditions shall be referred to the Contracting Officer for action. The COR writes the contractor’s annual CPAR.

Course Curriculum Model Manager (CCMM) - The CCMM is responsible for regular surveillance of the contractor's effort regarding compliance with CFET course performance. The CCMM POC will attend CFET training courses annually to assess training. The CCMM will conduct routine inspections of training records. The CCMM reports all surveillance findings to the COR for inclusion in the annual CPAR.

NMOTC Training Safety Officer (TSO) - The NMOTC TSO is responsible for regular surveillance of the contractor's effort regarding compliance with CFET course safety. The NMOTC TSO will attend training courses on an annual basis and assess training safety. The NMOTC TSO reports all surveillance findings to the COR for inclusion in the annual CPAR.

In-Service Engineer (ISE) - The COR will provide for an ISE inspection annually or as needed for device and equipment inspections. NAWCTSD will provide support as required.

4. Method of Surveillance. For all courses, the outcome shall be a successfully trained student.

Performance success is measured against standards established by:

CNAF M-3710.7. “NATOPS General Flight and Operating Instructions Manual”, dated 15 May 2022.

OPNAVINST 1500.75D. “Policy and Governance for Conducting High-Risk Training”, dated10 Aug 2017.

BUMEDINST 1551.4. Naval Aviation Survival Training Program Devices – Quality Assurance and Revalidation Policy, dated 24 Feb 2017.

NSTI-Approved Contractor-Provided CFET Training Curriculum.

The responsible Government surveillance inspectors will inspect performance by:

Reviewing contractor-maintained records Observing actual performance during site visits Attending training courses quarterly as an observer and monitor performance quality Reviewing student post-training evaluation forms Conducting annual on-site inspections

Surveillance shall not impact the contractor's successful accomplishment of the mission.

4.1 Observation/Inspection. Surveillance inspectors will usually inspect safety, training and equipment records using the High-Risk Training (HRT) Program Inspection Checklist per the

PWS. Inspectors will visit sites to make personal observations and may use other report or inspection formats as needed. The QASP matrix below delineates the elements by which the Government will measure the contractor’s performance. The Government observers or inspectors will deliver a copy of the results of the inspections to the contractor and to the COR for assistance in making corrections.

4.2 Student Feedback. The COR will review student course evaluation forms at the end of formal training sessions.

If the COR, CCMM, or NMOTC TSO receives student complaints, the COR will notify the contractor of the nature and severity of the complaint. The COR will refer complaints to the PCO if the contractor cannot resolve them within 24-hours of notifying the contractor.

5. Performance Deficiencies. When surveillance detects deficiencies in training services, the CCMM, the NMOTC TSO, and the COR will notify the contractor.

7. Unacceptable Performance. When surveillance detects ongoing deficiencies that the contractor does not address in a timely manner, the COR will notify the contractor. The Government may:

Require rework Require contractor to ensure future performance conforms to contract requirements Issue Cure Notice Issue Show Cause Terminate the contract for default List the issues in the annual CPAR

Method/Measure Incentives

Academic Classroom Instruction

100% of Terminal Objectives and 100% of Enabling Objectives are accurately covered using the NASTP CFET curricula.

Contractor maintains written record of objectives completion for each student.

COR and NSTI POC inspect written records that show completion for each student.

Higher CPARs ratings and award of future options if all instruction meets or exceeds standards.

Higher CPAR ratings for inputs and assistance to amend and improve the course’s safety and training.
