About this file

This document is a Quality Assurance Surveillance Plan (QASP) for a fire extinguisher inspection and maintenance contract for the VA Pacific Islands Health Care System. The contract involves annual inspections and six-year maintenance of 164 fire extinguishers across six locations in Hawaii, including facilities in Honolulu and Kaneohe. The QASP outlines the government's surveillance methods, performance standards, and evaluation criteria, with a 95% acceptable quality level for contractor performance.

The surveillance plan details specific monitoring approaches, including direct observation and 100% inspection, with performance ratings categorized as positive (no more than 1 incident of non-compliance), neutral (no more than 2 incidents), or negative (more than 3 incidents). The contract includes incentives for favorable performance evaluations and disincentives for unsatisfactory work. The Contracting Officer and Contracting Officer's Representative will oversee the contract, with monthly performance assessment meetings and the ability to issue Contract Discrepancy Reports for unacceptable performance.

a. Frequency of Measurement.

During contract performance, the COR will periodically analyze whether the negotiated frequency of surveillance is appropriate for the work being performed.
