Criterion 1: Technical Approach (70 points) Total Score: ______
The offeror shall identify methods and procedures required to complete each task. For sample tracking and storage and, include a description of the entire period; both before and after sample preparation, retention of samples and storage of re-test samples. Detail specific equipment for storage (# of freezers, temperatures and monitoring). For all the ingredients and tests proposed for analysis, identify specific methods and any modifications, and include limits of detection and quantification. Identify ingredient forms and DS forms (tablet, capsule, etc.) that can be analyzed with each method. Identify how analytical methods were validated for use in the specific laboratory to assure accurate results. Include descriptions of weighing and homogenization steps and steps to prevent contamination and deterioration of samples. Also specify how often this lab performs tests and analyzes ingredients in this proposal.

Criterion 2: Past Performance (35 points) Total Score: ______
Offerors shall provide information on past performance on contracts for the same or closely similar work called for in this solicitation which demonstrate an ability to meet the contract objectives. Companies will be evaluated on the extent and quality of past related experience and demonstrated competence. Offerors shall include the following information, as a minimum, regarding their past performance:
a) Past performance of personnel identified in criterion number 4. Include names and phone numbers of client project officers who would be able to comment on individuals’ past performance
b) A reference list of at least five previous business associates who can verify capabilities of the organization’s performance on specific projects.
