About this file
This is a Past and Present Performance Questionnaire template used to evaluate contractor performance on past or current contracts.
Section A requests general information about the contractor being evaluated, including the contractor's name, address, contact details, project title and work description, contract number, dollar amount, performance period, and the contractor's role (prime contractor, sub-contractor, or key personnel). Section B collects information about the respondent completing the questionnaire, including their name, title, address, telephone, fax, and email. Section C directs respondents to submit the completed form via email or fax to the requesting firm. Section D contains the performance evaluation criteria using a three-point rating scale: "A" (Acceptable) indicating the contractor will successfully perform required efforts based on its performance record; "U" (Unacceptable) indicating the contractor will not be able to successfully perform required efforts; and "N" (Neutral) indicating performance was not observed or is not applicable to the current effort. Respondents must provide narrative explanations for any "U" ratings. The evaluation addresses 21 performance factors organized into categories covering management and supervision, quality control, problem identification and resolution, staffing and schedules, responsiveness to changes, project supervision, personnel qualifications, subcontractor performance, cost proposals and invoicing, and overall performance rating. A remarks section provides space for additional comments.
