About this file
This is a Past Performance Questionnaire (PPQ) template used by the U.S. Senate Sergeant at Arms (SAA), Capitol Operations for evaluating contractor performance on past contracts relevant to Solicitation 2026-R-035 for Pipeline Leadership and Development Training.
The questionnaire is completed by references (contracting officers, project managers, or other government representatives) who managed contracts with the offeror. References must provide their company/agency name, contact information, role, and time period of involvement with the contractor. The form requests evaluations across eight performance categories using a six-point rating scale: Exceptional (EX), Very Good (VG), Satisfactory (S), Marginal (MG), Unsatisfactory (US), or Not Applicable (N/A). The evaluation categories cover fulfillment of contract requirements and specifications, schedule adherence, problem identification and corrective actions, contractor personnel expertise and quality, key personnel selection and retention, overall program management effectiveness, cost forecasting and management, and an overall assessment. Additional questions require the reference to indicate whether they would contract with the vendor again and to provide supplementary performance comments. Completed questionnaires must be returned within five business days of receipt to acquisitions@saa.senate.gov, Attention: Tonia Courtney, Senior Procurement and Contracting Specialist. The information provided is classified as source selection information and protected as procurement-sensitive.
